My overall goal for the Role Transition Practicum is to become more comfortable practicing independently, improve interpersonal skills, and to develop proficiency in nursing skills that are appropriate for my level of education and experience.

By the conclusion of this experience, the learning objectives I hope to achieve are to:

1.  I will independently prioritize patient care and complete a full nurse staff assignment. I will have achieved this goal if I have successfully completed a full assignment by the sixth week of clinical.

2.  I will increase my proficiency in basic nursing skills including taking vital signs, medication administration, proper documentation, urinary catheter insertion, dressing changes, tracheostomy care, ostomy care, and tube feedings. I will have successfully met this goal when I have safely completed all of the aforementioned tasks.

3.  I will independently document nursing care in clear and concise nursing notes. I will have achieved this goal when I have successfully completed documentation at facility standards.

4.  I will develop proficiency in nursing assessment. I will have completed this goal successfully when I have independently completed head to toe nursing assessments on at least three patients.

5.  I will improve patient interaction through development of therapeutic relationships with patients. I will achieve this through fostering trust and providing competent nursing care from admission until discharge.

6.  I will coordinate patient care by give oral report throughout the shift to both nursing staff as well as to the other members of the healthcare team. I will have successfully completed this objective when I have coordinated care for three patients within the six week rotation.

Present level of proficiency: My current level of proficiency is at the level of a senior baccalaureate nursing student. I can safely and accurately take a patient’s vital signs, perform patient hygiene tasks, administer medication, and assist a patient in safe ambulation and transfer techniques. I will need the assistance of a preceptor in the areas of administration of parenteral medications, suctioning, nasogastric tubes and feeding, patient admission and discharge, as well as urinary catheter insertion.

Future Goals: My future goals as a nurse include attending graduate school either directly after passing the nursing boards and achieving licensure as a registered nurse or after working on a medical-surgical unit for two to three years until I feel as though I have developed proficiency as a registered nurse. When I attend graduate school I intend to pursue a master’s degree as a pediatric nurse practitioner.
