Donation Receipt Guide for Company Events – Company Coordinator

PURPOSE:

The purpose of this procedure is to ensure receipts for donations given at company events are created and available to all donors who request a receipt for their donation. Use this form for events such as silent auctions, bake sales, chili feeds, etc. that are held in the workplace.

Important: The company coordinator should not give any advice about tax donation eligibility.

COMPANY EVENT COORDINATOR:

1. The company event coordinator will be provided with an electronic copy of the

“Donation Receipt from United Way of Benton & Franklin Counties” form for use at all

United Way fundraisers (i.e. auctions, raffles, chili feeds) held at their company.

2.  The coordinator will complete the Event Title and Event Date portion of the form before printing.

3.  The coordinator will make these forms available to all donors at the event.

4.  The donor will be responsible for completing all shaded areas of the form as follows:

o  Enter information in the Donated by, Address, City, State/Province, Zip/Postage Code, and Phone sections.

o  Circle Type of Donation.

§  If donating cash for a chili feed, circle “Cash.”

§  If donating an item to be auctioned like a gift basket, circle “Auction Item.”

§  If donating food for a luncheon or auction item, circle “Food.”

§  If the donation is other than these, please describe the donation in “Other.”

o  Description of Donation: Describe the donation for all categories except “Cash.”

o  Fair Market Value (FMV) of Gift: provide the fair market value of the item donated. For example:

§  Used Item: determine the “Thrift Store Value” of that item.

§  New Item: Use the invoice/receipt value.

o  Fair Market Value (FMV) of Goods Received.

§  For donations of $75 or more given to purchase an item (i.e., for an auction, raffle), the Internal Revenue Service requires additional information.

§  Enter the FMV of what was received for the donation (golf game, dinner, gift basket, etc).

o  If you didn’t receive anything of value in return for your gift, please check the final box titled, “**Check box if no goods or services were provided in return for this contribution.”

o  The donor will be responsible for attaching document(s) demonstrating the FMV of goods or services donated or received.
