JOB TITLE:Health Supports Educator (RN)

JOB SUMMARY:The Health Services Educator (RN)position:

  • educates and trains non-licensedteam members on health and wellness issues related to individual care, and to supervise the self-administration of medication, and
  • deems non-licensed team members competent to assist program participants with identified health maintenance activities, and
  • provides after hours on-call phone support for UCPGA homes in the East Georgia region (Augusta & Savannah).

The Health Services Educator’s goal is to educate direct support professionals well so that they may ensure program participants receive the highest level of quality medical care.

EDUCATION OR TRAINING REQUIREMENT:Bachelor of Science in Nursing (BSN) preferred. A minimum of an Associate’s degree or diploma in Nursing from an accredited program is required in conjunction with a current licensure as a Registered Nurse (RN).

EXPERIENCE REQUIREMENT:

  • Minimum 5 years experience of clinical nursing practice.
  • Minimum 2 years experience in the field of developmental disabilities.
  • Minimum 2 years experience in a position providing patient education.
  • Working knowledge of funding, licensing and accrediting body regulations and standards.

SKILLS REQUIREMENT:

  • Strong communication skills to educate lay persons on medical conditions and following plans of care.
  • Ability to adequately prepare lay persons to assist with health maintenance activities in program environments.
  • Knowledge of adult learning theory and principles.
  • Exceptional organizational skills to assist with development of curricula that meets learners’ needs.
  • Ability to assess health situations telephonically and communicate next steps to lay persons succinctly.
  • Ability to build shared understanding of – and shared commitment to – common goals.
  • Ability to function as a strong team member by demonstrating core characteristics of openness, trust, support, and respect.
  • Monitoring/Assessing performance of self and other individuals to make improvements or take corrective action.
  • Giving full attention to what other people are saying, taking time to understand the points being made, and asking questions as appropriate.
  • Communicating effectively verbally and in writing as appropriate for the needs of the audience.
  • Knowledge of basic computer concepts and usage of computer systems and software (Windows OS, Microsoft Office, email, etc.).

TRAVEL REQUIREMENT: Estimated required travel for this position is approximately 30%.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  1. Maintains a working knowledge of funding, licensing and accrediting body regulations and standards related to medical services, and the internal documents that meet these requirements.
  2. Maintains a working knowledge of UCP of Georgia policies and procedures related to medical services, and participates in regular policy/procedure development, review and revision.
  3. Provides training for new employee orientation monthly using UCPGA’s established curriculum, and other small group or individual training sessions related to health services as needs arise.
  4. Provides Proxy Caregiver competency observation to include evaluation of staff members in UCPGA’s East Georgia program locations.
  5. Evaluation includes observation of competency in demonstrating health maintenance activities and knowledge of medication assistance procedures, and routine evaluation of continued competencies, at least annually, if not assessed more often as defined in an individual’s Written Plan of Care.
  6. Evaluation of competency completed in accordance with the Official Code of Georgia Annotated (O.C.G.A.) §§ 31-7-2.1and 43-26-12, the Georgia Department of Community Health (DCH) policy titled Rules and Regulations for Proxy Caregivers Used in Licensed Healthcare Facilities, and the UCPGA policy titled Proxy Caregivers.
  7. Evaluations are to be completed for identified new hires, when there are significant health changes as defined by DCH’s policy, and at least annually for current staff members.
  8. Gathers health information and uses it to complete Written Plans of Care for individuals identified as requiring Proxy Caregivers.
  9. Completes administrative tasks related to training and competency observation including, but not limited to, scheduling training sessions, setting up class meeting spaces, grading tests, and completing forms necessary to record class completion for learners.
  10. Provides after-hours, weekend and holiday telephonic on-call duties for East Georgia program locations, to include documentation of calls and telephonic guidance provided.
  11. May provide assigned training and/or proxy caregiving evaluation outside the East Georgia region if needs arise.
  12. May participate in case staffing, program reviews, committee assignments and other meetings as necessary and assigned.
  13. Maintains objectivity in position in order to set appropriate limits while working with staff & program participants.
  14. Develops and maintains positive and productive relationship with coworkers, community members, those individuals served, and their families.
  15. Supports consumers in their personal growth and development, respecting cultural, ethnic, spiritual and individual differences.
  16. Adheres to all agency policies and procedures.
  17. Other duties as assigned.
