UNIFORM CRITERIA
Medical Staff
Uniform Criteria have been established at RenownRegionalMedicalCenter for procedures that cross specialties. Therefore, you must submit the required documentation (as outlined below) when you request the identified Uniform Criteria privileges.
This documentation must show the patient’s name or chart number and may be in various forms: OR records, discharge summaries, progress notes or orders. In addition to the Uniform Criteria documentation, please carefully read the privilege card for necessary documentation and letters of support.
FOR INITIAL REQUESTS
Paracentesis, Thoracentesis, CVC, Lumbar Puncture, Arterial Lines, Arthrocentesis, and Swan-Ganz Catheterizations: If the applicant has completed a residency within the last five years, the applicant may obtain a letter from the residency director stating that the applicant is adequately trained and capable of performing said procedure/s. Otherwise the criteria are as follows:
Paracentesis
Two procedures within the last two years.
Thoracentesis
Two procedures within the last two years. May utilize thoracotomy or pericardiocentesis procedures for documentation towards thoracentesis.
Central Venous Cannulation (CVC)
Ten procedures within the last two years – Swan Ganz will count toward CVC. If a physician has completed postgraduate education in the field of Anesthesia, Internal Medicine, Emergency Medicine, Family Practice or Surgery, it will not be necessary to supply documentation for CVC.
Lumbar Puncture
Five procedures within the last two years.
Arterial Catheter
Ten procedures within the last two years. Angioplasty and intraoperative arteriogram will count toward arterial catheter.
Joint Aspiration(Arthrocentesis)
Five procedures within the last two years.
Swan Ganz
Five procedures within the last two years (minimum of three for insertion; maximum two for monitoring); right heart catheterization counts toward numeric documentation.
Flexible Sigmoidoscopy
Ten procedures within the last two years, needs a mentor or demonstrate completion of didactic training and be mentored for ten cases by a mentor. Privileges include cold forceps biopsy. (7/98--rev. 6/99)
Ventilator and BIPAPTherapy
Applicant must request privilege and state he/she knows how to use ventilators. Applicant out of residency must have a letter from his/her program director stating he/she knows how to use ventilator plus documentation of ten cases from the last two years. BIPAP therapy may only be performed in the ICU’s.
Microvascular Surgery
- Demonstrate fellowship training or significant postgraduate training in Microvascular surgery.
- Document at least 30 microvascular reconstruction cases.(12/96)
Anterior Arthroscopic Spine Surgery
Have Thoracoscopy, Laparoscopy, and Vascular privileges. (12/96)
