UKStart-up form

This form provides an opportunity to develop your project aims and the plan set out in your application. It will act as a reference point for the future reports whichwe ask you to send to us every 12 months to tell us about your progress (and a shorter update every 6 months). These reports allow you to reflect on your progress and share your learning with us, and we use them to check you’re on track. We also use this information together with that from other projects to understand what difference we are making and to help us raise money.

Please refer to the guidance notes to understand exactly what we are looking for in each section and how much information to provide. You will also need to refer to your application form. Please note, fields highlighted in blue should be copied from your application form, as specified. When you have completed this please upload the document to GEM and if you have any problems, please contact your grant manager.
