2012-2013 FAMU FEEDER FELLOWSHIP ACCEPTANCE AGREEMENT
I, ______, an enrolled, or a to-be-enrolled, student at The Florida State University, in the Department of ______, having made application for and been awarded a Fellowship from The Florida State University beginning Fall 2012 and for other good and valuable consideration, do hereby agree to the followingupon acceptance of the award for the period of time of such award:
Initial: ______1) I will continue to be enrolled as a student at The Florida State University for the required number of semester hours as specified by the Department or The Graduate School, that number of hours being 12;
Initial: ______2) In order to continue to qualify for receipt of this Fellowship, I will maintain good academic standing, the required Enrollment, Grade Point Average (GPA) and Semester GPA as set forth by The Florida State University and Department or The Graduate School;
Initial: ______3) By my signature below, I hereby accept the Fellowship and all conditions, provisions and terms associated therewith;
Initial: ______4) In the event I drop out, discontinue, terminate or suspend my enrollment and/or registration at The Florida State University for any reason, or if The Florida State University terminates my enrollment, I understand I am obligated, responsible and liable for, and hereby specifically agree to be obligated, responsible and liable for immediate repayment of the un-entitled portion of funds already paid; and
Initial: ______5) By my signature below I agree to allow The Graduate School to use my name and/or photography on the Florida State University website for listings and articles pertaining to recipients of graduate fellowships.
Initial: ______6) If during the semester, I drop below the required number of hours I will notify The Graduate School immediately as my award is contingent on my continued enrollment as specified in #1 herein.
Initial: ______7) Furthermore, in the event that I do not make immediate repayment of any unentitled portions of funds already paid as set forth in #4 herein, I understand and agree that I will be subject to any and all actions provided by statute and Rules of the Florida State University and Florida Board of Governors, as well as, in the event of litigation, I will be obligated, responsible and liable for principle and statutory interest accrued to the unentitled portion of funds already paid, and for all costs and expenses incurred by the university in collecting this amount, including attorney fees.
Initial: ______8) Asa fellowship recipient, I understand that I will be expected or be eligible to participate in the Fellows Society organized by the Graduate School.The goal is to facilitate networking and professional development for recipients of university-wide fellowships.
Initial: ______9) I understand that this fellowship may only used for academic work in the department that nominated me and is non-transferrable.
Initial: ______10) I understand that I will be put on probation by The Graduate School if I earn an incomplete in a course and The Graduate School will not issue a tuition waiver or fellowship payment until the incomplete is satisfied.
This agreement is to be construed pursuant to the Laws of the State of Florida and the Rules of The Florida State University and Florida Board of Governors. NOTE: This agreement is not valid unless signed in original ink by both parties.
IN WITNESS THEREOF, I have read and understand the foregoing, agree to be bound by the provisions therein, and freely and voluntarily execute this Agreement on this the ______day of______, 20______.
