Destination and purpose of trip: Yesterland Farms, Canton, TX
Leaving Date 12/14/17 Time: 8:30 Estimated Hour of Return: 1:45
I understand that the group will travel by bus.
Normal precautions will be taken in the interest of safety and well-being. It is understood however, that neither the Tyler Independent School District nor any employee or sponsor thereof will be held liable for any accident, injury, or illness that might occur.
The privilege to go on this trip carries with it the obligation for a student to conduct himself in such a way that credit will be reflected upon the student, the school, and the home which is represented. Correct dress and behavior for this particular trip will be observed.
In case of accident or sudden illness I authorize a representative of the Tyler Independent School District to refer the above named child to available
Medical service.
Further details: Cost per student is $10.00. Parents will have to pay at the front gate.
