PURPOSE FOR FORM 2942:

The information in Form 2942 will provide information regarding the facility wide raw materials, facility wide products, and facility wide emissions.

ITEM BY ITEM INSTRUCTIONS:

Several identical tables and/or blank rows may be provided in these forms to allow flexibility and to help ensure submittal of complete information and data. You may add additional tables in the document and/or rows in a table by selecting the “unprotect document” or “stop protection” function. The location and use of this function varies depending on your version of Word. (NOTE: password = baq.) You may copy and paste additional tables as necessary to list all of the equipment, processes, control devices, emission points, pollutants, emission rates, etc. Additional rows may be added to each table by using the tab key when the cursor is in the last cell of the bottom line, by right clicking on the mouse and inserting a row, or by copying and pasting a row. The forms “protect document” tool should then be reselected so that you may resume navigating through the forms with the “tab” key.

A. Application Identification

Please provide the information requested in this table.

1. SC Air Permit Number: The existing South Carolina Air Permit Number assigned by the Bureau of Air Quality. This number can be found on an existing permit.

2. Application Date: Please ensure that the Application Date is the same on all of the forms and required information submitted in this Title V Operating Permit application package.

3. Facility Name: The business name used to identify the commercial or industrial facility at the physical location.

B. Facility Wide Raw Materials

Please provide the information requested in this table.

1. Raw Materials Used: List all the raw materials used in the total process. Major raw materials are those which comprise 10% or more of the total process raw materials. Other raw materials shall be listed if they are a regulated pollutant or contribute to the formation of a regulated pollutant.

2. Quantity Used Annually: The annual throughput for the facility processes (i.e., major raw materials are those which comprise 10% or more of the total process raw materials). Please indicate the units of measure.

C. Facility Wide Products List

Please provide the information requested in this table.

1. Products Manufactured (List in order of major to minor: List a brief description of the products as identified by SIC and NAICS codes, including any associated with each alternate scenario identified by the facility.

2. SIC Code: The SIC Code for the product provided by this facility. Standard Industrial Classification Codes (SIC Codes) are four digit numerical codes designed by the U.S. Department of Labor in order to create uniform descriptions of business establishments. The major SIC Code is assigned on the basis of its primary activity. (http://www.osha.gov/pls/imis/sic_manual.html))

3. NAICS Code: The NAICS Code for the products manufactured by this facility. North American Industry Classification System (NAICS) Code, a six digit coding system, is a series of number codes that attempts to classify all business establishments by the types of products or services they provide. 4. Annual Production Rate: The production rate on an annual basis for the products. Please indicate the units of measure.
