GUIDANCE NOTES

Note A – Provision of this information is needed to confirm your existing entitlement to grandfather rights. Not providing it may materially affect whether Acquired Rights are granted.

Note B – Please number each sheet. For example, if you have worked for one company please state 1 of 1. If you have worked for 3 companies please number as each page as 1 of 3, 2 of 3 and 3 of 3.

Note C - Please provide the full name of the operator, if possible as quoted on the operator’s licence.

Note D–The operator does not need to hold an O-licence if your role conformed with the roles and responsibilities of a transport manager, as set out in Guidance Note 6 below.

Note F – This should include:

  • Details of the roles and responsibilities undertaken at each operator you worked for in the period from 4 December 1999 to the present.You may wish to highlight some or all of the following areas:
  • Your role in ensuring that drivers comply with drivers’ hours and tachograph rules and with speed limits;
  • Your role in ensuring that vehicles were maintained properly, including the inspection of vehicles, action taken to remedy defects found and records;
  • Your role in ensuring vehicle defects found by drivers were reported and recorded properly;
  • Your role in ensuring vehicles were not overloaded;
  • Your role in ensuring that the relevant traffic commissioner was informed in writing about all prosecutions and convictions concerning the operator, the drivers and you.
  • You may find it useful or simpler to provide supporting documentation – such as copies of letters or contracts of employment or copies of job descriptions. If you provide these or any other documentation as evidence of the areas of responsibility outlined above, you can simply state "refer to attached supporting documentation".
