Job Title: Transitions Coordinator

Job Purpose: To develop and support the county wide transitions project entitled Time 4 Us

in co-ordination with 5 Project Workers and Dare2Dream thus ensuring that

being a Young Adult Carer (YAC) doesnot impact on their potential

attainment and aspirations.

Fixed Term Contract: Two Years

Based at: Carers Support Service, Ryton Gardens, Wolston Lane, Ryton on Dunsmore,

Warwickshire, CV8 3ES

The post will involve travel across Warwickshire to carry out this role.

Reports to: Chief Executive

Salary: £23,166(NJC scale point 26) plus 6%pension supplement. Child Care Voucher

Scheme

Hours: 37 hours per week but the post holder must be willing to work flexibly as some weekend and evening work may be necessary

Holiday Entitlement: 28 days + statutory holidays

Key Responsibilities and Accountabilities:

  • Work with the 5 Project Workers to establish an organisational framework in delivering the service to Young Adult Carers (YAC’s)
  • To facilitate the mechanisms by which the Young Carers and volunteers have a voice and take part in the management of the project.
  • Work with YAC’s on the Steering Group and other identified YAC’s to establish the content for the provision of monthly information and guidance (IAG) sessions held throughout the county
  • Build relationships with local employers to offer input to the IAG sessions, work experience and apprenticeships
  • Liaise with the 5 Project Workers to identify participants for the Dare2Dream personal development courses.
  • Collate evaluation information from courses and Projects Worker utilising the baseline and review evaluation tool
  • Facilitate bi-monthly meetings and informal discussion with the 5 Projects Workers to discuss evaluation and identification of blocks and barriers to YAC’s to achieving their aspirations.
  • Support YAC’s to engage in training and skills development activities, such as training in 'life skills' – self- care, money management, preparing CVS or applying for jobs etc., – or through opportunities volunteering; work experience.
  • Refine the Career Guidance Tool in partnership with the National Institute of Adult Continuing Education (NIACE) and build an online information resource for Young Adult Carers relevant to their needs
  • Raising awareness of the needs of YAC’s and supporting good practice in the treatment of these young people amongst other service providers
  • Facilitate YAC peer support activities
  • Facilitate bi monthly meetings with the 5 Projects Workers to collate feedback

Service Development and Coordination

  • Work with the Project Steering Group to fully develop the Time 4 US project plan
  • Maintain strong and regular links with relevant staff from Warwickshire Young Carers Project and work closely with the 5 Project Workers with this service to deliver project outcomes.
  • Build and develop relationships with academic and pastoral staff within further education establishments to improve Young Adult Carer support which is sustainable in the future
  • Develop an effective and sustainable model of transition support for YAC’s entering further education
  • Map local provision and create links with existing services to ensure effective transitions planning for YACs moving into Adult Carer Services and better coordination of services for young adult carers
  • Work with local partners to ensure that the young carers service and other organisations providing services to young people can network, share learning and ideas.
  • Maintain oversight on national and local developments, legislation and policy that could benefit YAC’s
  • To share good practice and learning regarding work with national and local networks and feeding outcomes into the National Forum for Young Adult Carers facilitated by NIACE for the Government

Mentoring Support

  • Recruit volunteer mentors who will support young carers aged 14-17 to manage periods of challenge and change.
  • Develop the operational implementation of the vetting, induction, supervision and support of volunteer mentors for the project, working within best practice guidelines.
  • Keep a register of project volunteers and records of mentoring matches and mentoring activity and to complete all other administrative tasks as required.
  • Recruit and support other volunteers to support beneficiaries and project activities as the need arises.
  • To share good practice and learning regarding work with YAC’s and with mentoring activity.

Management and Reporting

  • To prepare quarterly activity reports for the Board of Trustees and the Time 4 Us Project Steering Group and the reports need for funders.
  • To ensure appropriate communication with the Chief Executive at all times, particularly regarding matters of Safeguarding Children.
  • To attend regular team meetings and supervision sessions with line manager.
  • To liaise with the CEO and Financial Controller over project expenditure.
  • To undertake any other reasonable duties after consultation as deemed appropriate to the role and task.
