Transition Workgroup Meeting Notes

11/18/08 10am-12pm

Workgroup members present: Gina Gembel, Valerie Curren, Sandy Lane, Karla McCandless, Lisa Truscott, Carole Keefe, Kathy Stiffler.

Workgroup members on phone: Ellyn Samuels, Lori Skrzysinski, Melanie Dunsmore, Violanda Grigorescu

Objective 1: Create additional services to cover young adults including, health care coverage, a CSHCS buy-in program, pharmacy coverage, and mental health services.

  • Discussion began with the topic of the CSHCS Buy-in, it was noted that this seemed to be the section of the recommendation that strategic planning participants were voting on.
  • If a CSHSC buy-in was created there would be many things to consider including, whether the public health code would need to be revised to include the buy-in population, deciding on Buy-in time frames, coverage, and diagnosis based eligibility.
  • If a CSHCS buy-in was created you must think about staff time and how staff intensive such a program would be to run.
  • Idea was brought up about the possibility of partnering with Blue Cross Blue Shield of Michigan (BCBS) and their Young Adult Blue individual insurance policy. Could CSHCS possibly pay the premium for this coverage?
  • Could CSHCS also partner with a prescription assistance program for additional coverage for the CSHCS age-out population?
  • Could we do a pharmacy only buy-in?

Objective 3: Create standard requirements for transition planning with plans that begin at age 14 and are reviewed annually. Expand who would be eligible to bill for care coordination for such services.

  • Discussion on who else should be at the table for this topic. Invite: Michigan Rehab Services Rep, Adolescent Health Rep, Dept. of Education Rep, and youth advisors.
  • Decision that this topic should wait until we have a full group with all stakeholders at the table.
  • When standard requirements are in place it would be helpful to create a flow-chart in writing with all these requirements.

Objective 5: Identify all available transition services and establish a single point of entry.

  • This objective should be carried out in partnership with the Community Based Services workgroup as they have similar recommendations.
  • Could this single point of entry be somewhere on-line as opposed to a physical space?
  • The Kellogg foundation has a grant that they are working on with similar goals. Kathy Stiffler will locate the grant and see if there would be any opportunity to partner.
  • Could the single point of entry be Department-wide, for the Dept. of Community Health?
  • We need to survey what is currently available on-line for services and also research what other states have done.

Objective 2: Enhance collaboration between organizations and agencies that serve youth and adults.

  • It would be helpful for group members to know what is currently available
  • It would also be helpful for group members to know who our partners in strategic planning are.
  • See Action Items: List will be created listing all partner agencies/organizations that are involved in the strategic planning process
  • See Action Items: Draft a list of state and local agencies that serve young adults. This list will be send around to members for any additions.
