  1. With parent’s consent, an initial meeting to be held between agencies in question (ICDP, ICCP, LE) to share common client lists of children eligible for school the following September. A Lead is assigned at this meeting for each child.The Lead service provider carries out an administrative function to ensure all steps of the protocol are completed.
  2. The Lead initiates discussions with the parents to determine their plans/goals regarding sending their child to school the following year. The protocol is shared with them including discussions on the importance of timelines. The Lead assists the parents in determining what they would like assistance with, and who they would like assisting them. This Transition to School Facilitator (TTSF)may be the Lead or may be another person/service provider who knows the child well.
