Transcript Information Transcripts should be requested from Mrs. Gibson, by completing the gold “Request for Transcript” form available in the Guidance Office or downloading the Transcript Request form from the website. All seniors requesting transcripts must pay a one-time $2.00 fee. This covers all transcripts needed during your senior year and must be paid with your first transcript request. Please check due dates CAREFULLY, and plan ahead!
College and Scholarship Application Documents that must be completed by a school counselor must be submitted to Mrs. Gibson and/or your counselor at least five (5) school days before postmark deadline. The Guidance Office needs at least 5 days to make sure that all materials are completed and signed, a transcript is printed, and item is mailed by the due date. Please check due dates CAREFULLY, and plan ahead!
Any college or scholarship application document, due less than five (5) school days from the day that you submit it to the Guidance Office and request a transcript, is not guaranteed to be prepared on time.
Letters of Recommendation are not entitlements; they are earned. They should be politely requested, in person, at least two weeks before the deadline, from someone who knows you. It takes time to craft a well-written recommendation that reflects the uniqueness of the student.
Please provide, in writing, 1) to whom it is written, 2) for what purpose it is being written, 3) does the reader of the recommendation provide any instructions? If so, include them, 4) provide the deadline, 5) provide an envelope with the reader’s name/address, etc., 6) if the writer is to comment on your participation, honors, awards, etc., provide a list of your participation, honors, awards, etc. Say “Thank you”.

Standby Testing If you miss the registration deadline for a test date, you can try to test standby. Instructions for standby testing are available online at actstudent.org. If you are already registered for a previous or future test date, try to make a Test Date Change instead. Standby testing may be available at test sites on a first come first served basis for each test date.

The Legacy Financial Group Scholarship program is designed to raise the awareness of financial responsibility in high school and college students. The $1000 scholarship is available to students entering their freshman or sophomore year at an accredited Iowa college or education institution in fall 2013. The Jason Ness Memorial Scholarship is open to senior students of UHS with a record of service to Urbandale High School and the community. Applications are available in the Scholarship Drawer in the Guidance Office and must be submitted to Mrs. Gibson in the Guidance Office by May 6, 2013.
The Northwest Des Moines Rotary is making available a $1,000 scholarship to a graduating senior at Urbandale High School, Des Moines Christian High School or Metro West Learning Academy who is planning further education beyond high school. Applications are available in the Scholarship Drawer in the Guidance Office, and must be returned to your counselor by April 30, 2013.
The Winter Dance Party Scholarship of $500 is offered to graduating Iowa high school seniors planning to major in music. Information and applications are available in the Scholarship Drawer in the Guidance Office Applications must be received by April 15, 2013.
The Leo H. Grether Memorial Scholarship in Choral Music Education is offered to Iowa high school seniors planning to major in vocal music. Information and applications for this $1000 scholarship are available in the Scholarship Drawer in the Guidance Office Applications must be received by April 15, 2013.
The Urbandale American Legion Auxiliary Merit Award is accepting applications. Applications are open to students who are related to a veteran. Applications are available in the Scholarship Drawer in the Guidance Office. Submission deadline is April 21, 2013.
The Community Foundation of Greater Des Moines administers two tuition scholarships for local African-American students seeking higher education: the Mary E. Wood Scholarship Fund and the Larry W. Carter Scholarship Fund. Applicants can find additional information and download application forms at www.marywoodscholarship.com and www.larrycarterscholarship.com. Applications are also available in the Scholarship Drawer in the Guidance Office. The submission deadline is May 15, 2013.
The Beauty Schools Directory awards a $2,500 scholarship every three months, to aspiring Iowa students of cosmetology, barbering, esthetics, nail tech, makeup and other beauty disciplines. The Fellowship of Christian Athletes is offering a $250 scholarship to an Urbandale senior. Applications are available in the Guidance Office and must be submitted by April 26, 2013.
Beta Sigma Psi National Lutheran Fraternity at Iowa State University awards three $1,500 Darryl Jahn Memorial Scholarships to incoming Lutheran men. Beta Sigma Psi is based upon three core values: spiritual, scholastic and social. By being a member of this fraternity, men are encouraged to grow in these three values and also gain leadership experiences. Information and applications are available in the Scholarship Drawer in the Guidance Office. There are three application deadlines for the three scholarships: February 22, April 19, and May 31, 2013.
The Vietnam Veterans of America, Chapter 490 in Des Moines, offers two (2) $500 awards from the Kit Williamson Memorial Scholarship Fund to assist the children/grandchildren of Vietnam era veterans. Information and applications are available in the Scholarship Drawer in the Guidance Office and must be submitted by April 30, 2013.
The 2013 Student-View Scholarship Program is offering $10,500 worth of scholarship to high school seniors who plan to attend a four-year college, community/junior college or career school. Log on to www.student-view.com and complete a 15-20 minute application survey about colleges in your area. The scholarship will be awarded via a random drawing of all completed applications. The scholarship deadline is April 21,2013.
The Robert D. Blue Scholarship, made possible by the Iowa Centennial Memorial Foundation, awards scholarships to students planning to attend an Iowa college or university who demonstrate literary and scholastic ability, exhibit qualities of truth, courage and fellowship, and display moral force of character. The Iowa Section PGA of American annually offers the $1000 renewable Charlie Burkart Scholarship. Scholarship winners are selected on the basis of their interest and activity in golf and the potential for future contributions to the game. Abbott and Fenner Business Consultants’ scholarship program will award up to $1,000 each year. Students will submit an essay on the topic that appears on the scholarship page of their web site: http://www.abbottandfenner.com/scholarships.htm. Deadline is June 14, 2013.
The Iowa Masonry Educational Foundation provides assistance to Iowa students pursuing careers in masonry and construction-related fields. AES Engineers is pleased to be able to renew their scholarship offer. Applicants are not required to be taking Engineering courses to be eligible. The High School Abuse Prevention Magazine will award a $500 scholarship to a graduating senior with each issue (fall & spring) they publish in Iowa.
