Description of Placement

This is a placement in a psychiatric unit in Greater Manchester as part of a team of a consultant, core trainee and foundation trainee. The unit is situated within the grounds of Tameside general hospital and provides mental health care to the populations of Ashton-under-Lyne, Hyde, Dukinfield, Stalybridge, Denton and surrounding areas.
The mental health unit provides a variety of inpatient and outpatient mental health services including older age psychiatry, low secure rehab psychiatry and general adult psychiatry. Mental health input is also provided to the emergency department and general hospital wards.
Dr Earl team provides mental health care for working age adults living in the Ashton-under-Lyne area. Conditions commonly managed include schizophrenia, bipolar affective disorder, depression and personality disorders. Dr Earl also works with the local Community Mental Health Team (CMHT) and provides ongoing mental healthcare in the community. Outpatient clinics are held on-site at Buckton Unit and the CMHT are based at the nearby Haughton House in Ashton-under-Lyne. Psychiatric support is provided to inpatients in the General Hospital medical and surgical wards.

Roles and Responsibilities of the Trainee

·  Keep up-to-date with training needs and portfolio
·  Attend compulsory training programme (regional and local teaching programme)
·  Management of mental and physical health of inpatients
·  Attendance and contribution in ward rounds
·  Manage patients via outpatient clinics (approx. 1 new patient and 4 follow-ups a week)
·  On call and out-of-hours duties
·  To supervise and support the Foundation Year trainee
·  Collaboration and team-working with medical team, nursing team and CMHT

Core Clinical Work include details of where this will be carried out, type of assessments and therapies for which the doctor in training will be responsible (all experience must be appropriate to the level of training provided)

The trainee will be primarily responsible for the day-to-day management of approximately 10 psychiatric inpatients in collaboration with the foundation trainee. All newly admitted patients will need clerking and up-to-date documentation, mental state examination, risk assessment and full physical examination and blood tests. It is expected that all patients will have an ongoing weekly mental state examination and physical health review with further assessments or input as needed.
The trainee will be expected to attend and contribute to all ward rounds including feedback of weekly assessments, documentation, medication reviews and discussions about management. The trainee will be responsible for completing tasks and management plans as per ward round. On discharge from hospital there will be a timely and accurate discharge letter to the GP.
The trainee will have 2 outpatient clinics a week including one new patient and up to 4 follow-up patients. Initially all patients seen in clinic should be discussed with Dr Earl. Most patients are managed by a care-coordinator from the CMHT and it is expected that the trainee will provide support to CMHT members in the management of community patients where appropriate.
The trainee will be expected to carry out mental health assessments (under Dr Earl’s supervision) on the general adult wards for non-urgent consultation requests.

On Call

Participate in the on-call rota, which is 1:10 partial shifts and will involve day time on call at Tameside as well as nights, covering all acute psychiatric and old age wards in Tameside, the A&E department, Child & Adolescent Psychiatry emergencies (supervised by the on-call Child & Adolescent consultant), and the assessment of deliberate self-harm patients with the A&E liaison nurses.

Facilities

There are two offices on-site for junior doctors including IT access. There is access to admin support via Dr Earl’s secretary. There are catering facilities available at the general hospital. Parking is available for a fee.

Supervision Arrangements

Formal supervision will take place for one hour a week. There will be opportunity to discuss clinical cases, personal development and complete portfolio work. The trainee will be expected to take a proactive approach to supervision including self-directed learning.
This is in addition to ad-hoc informal clinical supervision during ward rounds, clinics and for urgent clinical management as required.

Trainees have the opportunity to shadow the Consultant on home visits Mental Health Act Assessments and attendance at MHRT and managers meetings.
Health & Safety:
You are provided with a personal safety alarm – wear it and use it. Ensure you use the safe interview facilities within the department. You are encouraged to go on a Managing Violence and Aggression Course within Tameside. Risk Management training also provided in the Induction Course.
