TOP DOG Walking Services are licensed by Bracknell Forest Council to provide Home
Boarding Services for Dogs and are Insured with Cliverton Insurance.
Dog home boarding/Day Care within a family home is a far less stressful experience for your pet
than staying in Kennels. We provide a calm home environment and a comfortable,safe and
friendlyatmosphere. Our family will welcome your Pet into ourhome and we will provide company
cuddles and playtime for your dogand a sociable companion with our own dog, we will also
exercise your dog while in our care and this is included in our Package.
All Dogs must be well socialised, obedient, well behaved and house toilet trained.
We regret that we are unable to accept bitches whilst they are in season.

Terms and Conditions of Boarding/Dog Sitting/Day Care
* Please note that confirmed bookings are taken with a 50% deposit of the total boarding fee -
payable by cash or bank transfer upon booking.Bookings will remain provisional for 48 hours or
upon receipt of the booking fee.Bank details available on request.
* Should payment not be received within 48 hours of booking, the bookingwill be cancelled and the
space offered to another Client.
* Please note that deposits are non refundable once bookings have beenconfirmed and are
subsequently cancelled by the dog owner.
* The remaining balance of the boarding fee will become due a minimum of14 days prior to the date
of boarding.
* Cancellation by us - In the unlikely event that we are unable to complete a confirmed booking for
any reason, we will refund your booking deposit in full and without any liability whatsoever or
obligation to provide you withalternative Care or boarding.
* Your Dog/s must haveit’s/their ownhealth and Liability insurance and a photocopy of the Policy
will be required.
* Dogs must be fully vaccinated against Canine Distemper, Infectious Canine Hepatitis (Canine
adenovirus), Leptospirosis (L. canicola and L icterohaemorrhagicae) and Canine Parvovirus and
other relevant diseases. The course of vaccination must have been completed at least four weeks
before the first date of boarding or in accordance with manufacturerinstructions. Dogs cannot be
boarded unless up to date vaccination records have been seen and photocopied for our records.
Worming and flee treatments should also be up to date.
* We alsorequest your dog is vaccinated for Kennel Cough as we cannot be held responsible
ifyour dog developsKennel Cough during it’s stay with us. Should your Pet contract Kennel Cough
whilst in our care it will not be possible to socialise it with any other dog for the remainder of the
stay. We also reserve the right to seek Veterinary Advice and administer medicine should it be
required.
* Please ensure your dog has a collar and contact name tag as this is a legal requirement, however
please be advised that in accordance with our Licence conditions your Pet will wear a tag with our
contact details for the duration the stay with us.
* Please bring your dogs bed/blanket and a favourite toy, this may help them settle more quickly,
however whilst great care will be taken with your dogs belongings, we cannot be held responsible
for any damage caused to beds, toys etc during their stay with us.
* You can provide their favorite food to cover their stay with us or if youwish us to provide their food
then we are happy to do so but this will incur an additional fee of £1.50 per day.
* Your Dog will be exercised within a group on a flexilead and will only be walkedoff lead where written
consent has been granted and oncondition they have good recall and without anyliabilitytoTop Dog
Walking Services and/or any of their Helpers.
* Dogs must be fully housetrained, well behaved, well socialised and obedient.
* We reserve the right to crate your dog if they show any unacceptable behaviour during their stay with us.
* Whilst every care is taken to make your dog’s stay a safe and enjoyable one, you should note that
dogs are left at their owner’s risk and no responsibility can be accepted for illness or injury, loss
through theft or straying or demise of the dog.
* The owners of the dog accepts liability for any damage caused by the dog to the boarding venue.

PLEASE NOTE THAT SHOULD YOUR DOG REQUIRE VETERINARY ATTENTION WHILST IN OUR CARE WE WILL DO OUR BEST TO GET YOUR DOG SEEN BY YOUR REGULAR VETERINARY PRACTICE. HOWEVER, IN THE EVENT OF AN EMERGENCY IT MAY BE NECCESARY TO USE ANOTHER VET. ANY COSTS INCURRED AT ANY PRACTICE WILL BE THE RESPONSIBILITY OF THE OWNER AND NOT THAT OF TOP DOG WALKING SERVICES AND FULL REIMBURSMENT WILL BE REQUIRED BY MEANS OF CLEARED FUNDS BEFORE OR UPON COLLECTION OF THE DOG.
Off lead exercise is only provided to those dogs whose owners have signed below. By signing below you are agreeing to allow us to exercise your dog off lead in areas we deem to be safe. Please note that whilst every care will be taken whilst exercising your dog, ALL dogs exercised by TOP DOG Walking Services are done so at their owners risk and no responsibility can be accepted for sickness, injury, loss through theft or straying or demise of the dog. Furthermore no responsibility can be accepted by TOP DOG Walking Services for any injury or loss caused to third parties or their belongings by the behaviour of a dog in our care.
CHARGES:
£30.00 -Boarding for each overnight stay (24 hours)
£25.00 – Doggie Day Care – per day (8am – 6pm)
£15.00- Dog Walking – for 1 dog, discount available for more than 1 dog per household
50% Deposit wlllbe required in advance to confirmthe Booking.
The remaining 50% balance will be due for payment 14 days prior to the commencement date of stay.
Payment by Cash or Same Day Bank Transfers please, bank details available on request.
Cancellation Charges
Cancellation up one month in advance 50% of total cost will apply
Cancellation up to two weeks in advance 75% of total cost will apply
Cancellation up to one week in advance 100 of total cost will apply

I hereby agree that I the undersigned agree to the Conditions of Boarding terms and conditions and give consent for Jeannette of Top Dog Walking Services to board and walk my dog as agreed for the period agreed in this booking form.
In the event of an emergency and if Jeannette is unable to make contact with me I give permission for Jeannette to seek veterinary treatment should it be required while in her care and I accept full liability for payment of any veterinary expenses incurred.
I also accept full liability for any loss or damage caused by my Dog whilst in her care and being walked and I accept liability for payment of any public liability insurance excess incurred.
In the unlikely event that TOP DOG Walking Services should have reason to cancel a booking this will be done as soon as possible via the contact telephone numbers providedby the customer and a full refund will be provided. I also understand that should this occur, I will not seek any form of compensation from TOP DOG Walking Services nor expect them to provide any alternative boarding arrangements.
You agree that these terms of use are fair and reasonable in enabling us to perform our obligations under this agreement to the best of our ability and to protect our legitimate business interests. I accept that I have read and accept TOP DOG Walking Services Terms and Conditions of Bookings.
