If you are unable to share a survey electronically, you can use this tool to gather feedback from your colleagues on your professional credibility.

Copy and paste the text below into a new document. Edit to indicate how and by when you expect to receive responses. Make sure to protect the anonymity of respondents.

Add your name at the bottom of the sheet. Print and distribute it to your colleagues.
