Subject:ANNUAL OPEN ENROLLMENT NOTICE

We will be conducting our Annual Open Enrollment from October 26 – November 20th.

This enrollment period provides an opportunity for benefit-eligible employees to enroll for the first time or make changes on their current plan in the following benefit plans: medical, dental, prescription and vision. The changes can consist of adding or dropping eligible dependents. Employees may also elect to change their premium contributions to a pre-tax or post-tax deduction and enroll in the flexible spending account for medical or dependent care. All changes and enrollments during this period will have an effective date of January 1, 2016. All forms must be received in the Human Resources office on or by November 20th.

If you do not make any changes to your plan, your current selections will rollover in to 2016. All employees with spouses or same-sex domestic partners with primary coverage under the University’s plan will be asked to recertify their coverage. Forms will be sent separately through the campus mail.No recertification will be necessary if your spouse or same-sex domestic partner has secondary coverage on the University’s plan.

A representative from FlexSave will be on campus to discuss the advantages of contributing to a federal tax free savings to cover out of pocket medical and dependent care expenses. The Flexible Savings accounts provide an opportunity to reduce your taxes and increase your take-home pay.You must re-enroll for each plan year. As per the IRS rules the maximum contribution cannot exceed $2,550 for medical health care and $5,000 for the dependent child care or elder care. You may use your FlexSave account for out of pocket expenses related to health care services that may not be covered by your health plan and dependent care or elder care.

Get ready for New Affordable Care Act reporting:

  1. Provide Information:We need specific information on all individuals enrolled in the health plan to provide you a complete 1095-C. We may be contacting you for necessary reporting information.
  2. Think Ahead:If you are benefit eligible for the Youngstown State University Health plan and receive a subsidy through an Exchange, you may be required to refund the subsidy.
  3. Heads Up: You will receive Form 1095-C for the first time in January 2016.
  4. Why Be Concerned: It’s important because you will need information on the form to prepare your 2015taxes. Watch for your Form 1095-C in January. It will be issued with W-2’s.
