Please complete and review this entire packet ofinformation, include a copy of your résumé (sample included in the senior packet), and return it to the College and Career Room B112 by April 6th. This information will be essential in helping us write any type of recommendation for you (scholarship, college applications, etc). ALL students are required to complete the above information.

The following needs to be completed as soon as possible by March 16, 2012 and returned to the senior packet collection box in the College & Career Room (B112) or return by email to Mrs. Kolb:

PRINT neatly using a black ink pen, or access these forms from the college and career website and complete on your computer:

  1. Complete your Personal Profile Form and your résumé, and return to the collection box.
  2. Have your parent/guardian complete the Parent Insights Form and the Transcript Release Form and return them to the collection box.
  3. Put your name on theTeacher Evaluation Form and give it to an academic teacher (math, science, social studies, English, foreign language) who knows you well. Ask your teacher to return the form to Mrs. Bond’s mailbox. If your teacher prefers, he/she may request that Mrs. Kolb email the document directly to him/her. You are not responsible for returning this form as many teachers will actually work on this over the summer.

Transcript Information: After you have applied to a college/university, in most cases application will be available on a college’s website after August 1st, please request your official transcript to be mailed to the college/university by contacting Mrs. Kolb at or by stopping by the College & Career Room (B112) and completing a request form. NO transcript can or will be sent from your high school (this includes coach’s requests) until the Transcript Release Form is on file in the guidance office, and NO transcript will be sent without a written request. After you have completed the release form and returned it, you may ask Mrs. Kolb (in writing or by email) to send your transcript. Your ACT and/or SAT scores, GPA, and class rank are noted on your official transcript. However, most colleges require an “official” test score to be sent directly from the testing agency. When registering for these tests please list your colleges of interest on your registration form so they will automatically receive your score.

College Visits:

Juniors and seniors are allowed two excused off-campus college visits per semester. Students must obtain a prearranged absence form from attendance orbring a signed parent note, or letter from the college admission office including the date and name of the college visited. Get these visits in early as a seniors’ college visit during second semester, senior year counts as an excused absence against your exam exemption.

Students are allowed to attend any HHS college private visit with teacher permission. These visits will usually occur in the College & Career Room B112. College visit dates and times will be posted to the College & Career website.

NCAA Eligibility:

Student-Athletes interested in playing Division I, II, or NAIA sports must be cleared through the eligibility centers for NCAA or NAIA. This process takes place online and requires “official” test scores from the testing agency, a transcript, and an application fee.

Scholarships/Financial Aid:

External/Community Scholarships will be posted on the HHS College & Career website as they are released to us. Most of these scholarships will be available during the spring semester of your senior year. Students should create a scholarship profile on a valid search engine like fastweb.com or scholarships.com to receive information about scholarships for which we may not have notification. Students should never pay to apply for financial aid or a scholarship opportunity.
