Immediate changes to the service forsouth Essex patients with suspect glaucoma

Dear Colleague,

As you will be aware, the cost of providing ophthalmic services in Essex is significant and has the potential to increase in the context of a population which is expected to grow, and within that the proportion of elderly people is projected to rise. This means increasing demand is being put on the care provided, the budget and the ophthalmic system as a whole.

In response to this, the south Essex Ophthalmology Clinical Network has developed several pathways with an intention to provide alternative options to the Hospital Eye Service (HES). Together these pathways form the Ophthalmology transformation programme that is being rolled out over the next year.

An early scheme that commenced in November 2015 was the expansion of the Glaucoma Repeat Readings service (GRR). Service users are identified through a standard General Ophthalmic Services Eye examination, privately funded eye test or by their GP and have an IOP >21mmHg with no other signs of glaucoma.

From Tuesday 29thMarch 2016 Southend Hospital Eye Service will no longer be accepting referrals for suspect glaucomaunless they have been through the community repeat readings service. Patients registered with GPs in Basildon & Brentwood, Castle Point & Rochford, Southend and Thurrock CCGs meeting the following criteria:

IOP >21mm Hg but <31 mm Hg with normal looking optic discs and visual fields

should be referred to one of the south Essex GRR Community Providers for repeat IOP readings to be taken prior to referral into the Hospital Eye Service.

Any referral received in Southend Hospital meeting these criteria will be returned to the referrer with advice to refer to the GRR providers for repeat IOP readings to be undertaken.

How to refer patients:

Castle Point & Rochford CCG or Southend CCGpatients

For patients registered with GP practices in Castle Point & Rochford CCG or Southend CCG the referral should be made directly to the provider (list of providers available in Appendix 1). The list of relevant GP practices can be found in Appendix 3.

The Local Optical Committee has developed a referral form that can be used for all referrals. This has been approved by the Hospital Eye Service and reviewed by the South Essex Ophthalmic Clinical Network. A copy is attached in Appendix 2. An electronic version of this form is also available on the LOC website at

It is good practice to offer patient choice; appendix 1 contains the contact details of the providers offering the GRR service. Please ensure your patient is aware they are being referred to this provider for the repeat pressure pathway only and will not have any other eye examination at this provider.

All GRR referrals should be faxed directly to the patient’s preferred provider using the standard referral form. A copy should be sent to the patient’s GP and marked “For Information Only” – Please note; GPs are no longer required to complete ophthalmic referrals.

Basildon & Brentwood CCG or Thurrock CCG patients

For patients registered with GP practices in Basildon & Brentwood CCG or Thurrock CCG the referral should be made via Evolutio(formerly known as Accipiter). The list of relevant GP practices can be found in Appendix 3.
