To All Parents:

Welcome to JAM’s Athletics Training Center, home of the Platinum Elite All-stars.

Allow me to introduce myself as Director Elizabeth Marsh. It is our pleasure to welcome you and your child to our family here at JAM’s Athletics. We look forward to the opportunity of providing quality service and to help provide a nurturing environment for your child while here at JAM’s Athletics.

We have a staff of dedicated “Coaches and Volunteer Parents” who have the desire to provide enriching, joyous, and fun filled experiences for you and your child. Our success lies heavily on the respect, cooperation and accordance we receive from you.

Please work with us so that Platinum can continue to be respected in our community, Atlanta, the State of Georgia, and nationally, as being a “First Class Operation”.

We would like to take this opportunity to introduce you to Platinum Elite All-Stars. We now hold over 100 National Titles.

Our mission is to promote physical fitness, discipline, leadership, social development and academic excellence through competitive cheerleading experiences. We are committed to our mission and we work hard to ensure a safe, fun and rewarding experience for each of our participants.

All-Star cheerleading is a competitive athletic endeavor. National competitions at this level are sometimes televised on ESPN and Fox Sports Network. During 2002, 2003, 2004 and 2005, divisions of Platinum Elite were televised as National Champions at the "World Cheerleading Association" competition held in Nashville, Tennessee. All-Star cheerleading builds more than just pyramids, it builds children. We offer each child an opportunity to build confidence and self esteem as well as develop the child's athletic skills and abilities. Cheerleaders at this level have an excellent opportunity to receive collegiate scholarships.

for injuries or illness he/she may incur while at the Stone Mountain facility or any event/function.

I authorize necessary medical treatment and admission to any hospital designated by Platinum Elite or there designate.

It is understood the parent or their agents will be called upon to give additional authorization if advanced treatment, (MRI, Lab Test, Surgical procedures, etc..) is necessary.

Our vision is to build a stronger child, a stronger family and a stronger community.

PHILOSOPHY

Our philosophy and top priority is building your child's confidence and self-esteem through a fitness-geared program. We believe in fitness and fun through our "Cheernastics" oriented activities. We believe that each child has the potential to be their own star in our program. The training of cheerleaders is our business.

Platinum Elite All-Stars is the best place for all ages and skill levels to participate in cheerleading. We believe that everyone is a star at Platinum. We are proud that our staff is Safety Certified and a GHSA member.

TEAM PHILOSOPHY

PLATINUM ELITE ALL-STARS, INC.

(RELEASE OF LIABILITY AGREEMENT AND MEDICAL WAIVER)

In consideration of being allowed to participate in any way in the Platinum Elite All-Star/JAM’s Athletic Competition program, Tumbling classes, Cheer technique classes, Cheerleading Boot Camp, Summer Camp, Arts & Crafts, and any related events/activities, the undersigned

acknowledges and fully understands that each participant will be engaging in activities that involve risk of serious injury, including permanent disability and death, and severe social and economic losses which might result not only from their own actions, inactions, or negligence of others, the rules of play or of the premises or of any equipment used. Furthermore, that there may be other risks not known to us or not reasonably foreseeable at this time;

Assumes all foregoing risks and accepts personal responsibility for the damages following such injury, permanent disability or death.

Releases, waives discharges and covenants not to sue Platinum Elite, it's affiliates, owners, their respective administrators, directors, agents, coaches, and other employees of the organization, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and leasers of the premises used to conduct the activities of Jam's Athletics, all of which are hereafter referred to as 'releases' from demands, losses or damages on account of injury, including death or damage of property, caused or alleged to be caused in whole or in part by the negligence or the releases or otherwise.

I HEREBY RELEASE AND SHALL DEFEND, INDEMNIFY AND HOLD HARMLESS THE RELEASES FROM EVERY CLAIM AND ANY LIABILITY that I or my child may allege against the Releases (including reasonable attorney's fees or costs) as a direct or indirect result of injury to me or my child because of my child's participation in the Platinum Elite activities/events and programs, WHETHER CAUSED BY THE NEGLIGENCE OF THE RELEASES OR OTHERS. I PROMISE NOT TO SUE RELEASES ON my behalf or on behalf of my child regarding any claims arising from my child's participation in the Platinum Elite All-Stars, Inc. programs, activities/events

Appearance Agreement: I understand that JAM’s Athletics Inc. d/b/a Platinum Elite All-stars from time to time produces promotional material relating to its programs. I understand that as participant may be included in videotapes, DVD’s, podcasts and video casts or photographs taken during the season.

Therefore, without reservation or limitations, I, in my own behalf and on behalf of Minor, hereby assign, transfer and grant to JAM’s Athletics Inc. d/b/a Platinum Elite All-stars, its successors, assignees, licensees, sponsors, any television networks, and all other commercial exhibitors the exclusive right to photograph and / or videotape Minor and to utilize such videotapes and photographs and Minor's name, face likeness, voice and appearance as a part of the Program or in any other media now in existence or hereafter developed, in advertising and promoting the Program, in advertising and promoting similar future events or in advertising and promotions relating to JAM’s Athletics, Inc. without reservations and limitations. I further understand that neither JAM’s Athletics, Inc. nor any third party is under any obligation to exercise any of the foregoing rights, licenses and privileges herein granted. I waive any right to inspect or approve the programs, copies thereof and any promotional materials related thereto.

PARENT PARTICIPATION

·  Parents and Participants are expected to display proper sportsmanship and attitude at all times. Participants will respect all adults and their peers.

·  Parents are expected to be role models and supporters of all cheerleaders and coaches in our organization. Repetitive misconduct or display of poor sportsmanship or attitude will result in dismissal from the program. ALL FUNDS PAID ARE NON-REFUNDABLE/NON TRANSFERABLE.

·  Distractions from parents/siblings and/or friends during practice will not be tolerated. We only have a limited amount of space in the gym and need to use practice time to prepare for competitions/performances.

·  Platinum asks for your support in allowing our coaching staff complete concentration during practices in order to optimize the time allotted for practice.

·  Parents are asked to follow the parking rules. Parents are not allowed to park their cars along the curb. Parents/Cheerleaders cannot double-park their cars.

·  Any parent complaints must be in writing and giving to the front office staff. The Director will contact you within 48 hours.

PARENT INFORMATION

·  Parents are required to pick up current information at the beginning of their child’s practice session. Please review the information.

·  It is the parent’s responsibility to make sure they check with his/her coach to ensure that they have updated information pertaining to the program.

·  It is the parent’s responsibility to make sure that all pertinent contact information is up to date, ex., telephone numbers – day and evening, address change, email address, etc.

PARENT CODE OF ETHICS AT THE GYM/COMPETITIONS

·  The use of Platinum’s and/or JAM’s Athletics name is strictly PROHIBITED. You may not put “Platinum” and/or JAM’s on shorts, shirts, hats, bags, etc. without the prior authorization from either Elizabeth Marsh. Such disregard will result in the removal of your child from the program.

·  Parents should avoid any loud or disorderly conduct. Any disagreements within the organization should be addressed to the director, not other clients.
