SUBJECT:Request for Health Insurance for Former Spouse’s Children
The following information is from the Defense Finance and Accounting Service (DFAS), Division of Services and Support’s notice dated March 28, 2002, on how to obtain health insurance for dependent children of a service member’s former marriage. Health coverage may be obtained for the dependent child(ren) through the Civilian Health and Medical Program for Uniformed Services (CHAMPUS).
In order for the service member’s former spouse to use CHAMPUS, the child(ren) must have a military dependent ID card issued for CHAMPUS use only. The service member may obtain the ID card. If he/she refuses to obtain the ID card, the former spouse can go to the nearest military post and report to the ID section, usually located in the installation’s Personnel Office.
The former spouse will need to present a marriage license, divorce decree, and birth certificate(s) for each child born of the former marriage. If there was a child(ren) born to the mother that the service member adopted during their marriage, he/she will need adoption papers for the child(ren). The former spouse must state that the service member refuses to process a DA Form 1172, Application for Dependent ID Card, and that the card(s) are for CHAMPUS purposes only. The ID card section will then review the documentation to verify the child’s right to the ID card and issue one without the service member’s compliance.
Each child’s ID card must be presented when seeking medical attention or treatment. The medical facility will then process the CHAMPUS paperwork for the services provided and any expenses not paid by the CHAMPUS program would be billed to the service member.
If the former spouse remarries and the new husband adopts the child(ren), all rights to CHAMPUS benefits will end and the ID card(s) must be returned. If the new spouse does not adopt the child entitlement to CHAMPUS continues.
Once the ID card is obtained, or if the former spouse experiences difficulty in getting an ID card, a complete explanation of CHAMPUS procedures can be obtained from the CHAMPUS

administrator at any military installation. A complete explanation of CHAMPUS procedures

Currently, the active member’s wages cannot be garnished for the purpose of obtaining health insurance for the child(ren). The military does not have a program in place to accomplish this. The member will have to be contacted directly to enforce wage withholding.
