SUBJECT:Revised Procedural Instructions for the Assignment of Rights and Authorization to Collect Support (Form CS-140), Notice to Contact Court(Form CS-142), Outstanding Check Letter (Form CS-172), Cashed Check Letter (Form CS-173), Outstanding and Cashed Check Letter (Form CS-174) and Lost or Stolen Check Affidavit (Form CS-176)

The procedural instructions for the CS-140, CS-142, CS-172, CS-173, CS-174 and CS-176have been revised to update the format and to reflect technical changes to be consistent with other procedures. The procedural instructions for the CS-140, CS-142, CS-172, CS-173, CS-174 and CS-176 have been updated on the CHFS Intranet Forms page.

In addition, the procedural instructions for the CS-140 have been revised to update the instructions for generating the CS-140. The procedural instructions for the CS-142 were revised to include the types of service of process and corresponding codes.

The procedural instructions for the CS-172, CS-173, CS-174 and CS-176 have been revised to clarify that replacement or duplicate checks are issued by the Kentucky State Treasurer, not the Accounting Branch.

This transmittal letter is to be saved and filed for future reference as it explains changes that have been made to the Kentucky Child Support Forms. Update as shown below and on page two.
