As you know, our group employee benefits will renew on January 1. After evaluating our current plans with other carriers in the market, we are pleased to announce that we will be keeping our current plans.

December is our open enrollment period. Any employee and/or dependent who have waived coverage may submit an application during this time for a January 1 effective date.

MEDICAL

Below is a brief description of our Christian Brothers Services plan along with your monthly premium. CB Services utilizes the Aetna network. Your account can be seen by logging in as a participant to Attached is the Summary of Benefits which describes the benefits in more detail.

HEALTH REIMBURSEMENT ACOUNT (HRA)

With the benefit of our TASC HRA, you are reimbursed all but the first $500 of your deductible by Vianney. As you know, when a claim is incurred, you receive an Explanation of Benefits from CB Services regarding your payment due to the provider. A copy of this EOB should be submitted to TASC along with a TASC Request for Reimbursement form. This can be done electronically or by fax. TASC will tabulate your claims and reimburse the employer’s portion directly to you once your responsibility towards your deductible has been met.

DENTAL

Our dental coverage is offered through Guardian. Guardian offers a large network of providers which you can access by clicking on “Find a Provider” through Log in to this website to access your account information and other services.

Our Guardian dental policy will renew with no increases this year! The following monthly rates will continue:

VOLUNTARY VISION

Vision is also offered through Guardian and will continue at the same 2016 rates for 2017. Vision insurance covers in-network exams and materials with copays. See the website information below for access to a more detailed summary.

Employer Paid Life Insurance

Vianney will continue to provide for each employee term life insurance with a benefit of 1½ times your salary through Guardian.

Voluntary Employee and Dependent Life Insurance

Extra life protection can be purchased for you and your family through Guardian. If you would like to make changes to your current coverage or elect voluntary life you should do so before the end of the month. If you are increasing your coverage or coming on to the policy after you were first eligible, you will be asked to answer a few health questions.

EMPLOYER PAID Long Term Disability (LTD)

Vianney also provides a long term disability policy for each employee. Should you be unable to work due to sickness or an accident, Guardian will begin reimbursing 60% of your earnings up to a maximum of $5,000/month after a 90 day “elimination period”.
