FEE PAYMENT POLICY
The total fee, along with a signed financial agreement will be due at tryouts to secure and guarantee a position on the team. Subsequent payments will be due based on the selected payment plan chosen by the parent who shall be attached to this agreement and must be followed accordingly.

Any payments received by the due date will be considered late and will not be accepted with out a $15 late fee.
_____1. I understand that I am paying for my child to learn the fundamentals of volleyball in an effective respectful environment. Victory coaches will provide a role on the team for every player but my child will not be guaranteed equal playing time in tournaments. Decisions concerning player participation in tournaments are solely the responsibility of the coaches.
_____2. My attendance at matches and tournaments is welcomed and anticipated. I will conduct myself in a respectful manner by:
· Showing appropriate sportsmanship towards all teams, parents’ fans, officials, coaches, and staff. I will refrain from loud or profane language.
· Being patient and supportive with athletes, regardless of their play.
· Accepting decisions made by coaches and staff.
· Discontinuing behavior deemed unacceptable at the staff, coaches, player’s officials or other fans. If I fail to honor such a request, it is within the right of Victory staff or any hosting organizations staff to request, it is within the rights of Victory or any hosting organizations staff to request that I leave the playing area or the building.
· Refraining from distracting my child, other players or coaches during competition.
___3. I agree to follow the Grievance Policy and Procedures as outlined; A 24-hour cooling-down period is required and enforced before any grievance is reported. Confronting a coach before, during or after a practice or game will not be tolerated.
1. Athlete asks the coach at an appropriate time to discuss any issue

3. Athlete and or parent/guardian and coach discuss problem with Coach Bailey
4. Meeting with all Board Members of Victory Volleyball
_____4. My child must remain at the competition site until released by the team head coach. If I remove my child from a competition site without prior permission, Victory will consider my child to have resigned from the club and all outstanding fees become immediately due and payable.
_____5. I am responsible for my child’s conduct; I will be contacted if a problem with my child arises.
_____6. I agree that I will be financially responsible for all fees assessed relating to my child’s participation in Victory. I will pay any penalties resulting from my failure to meet the terms for this agreement. If I remove my child from the club before the end of the season I am still obligated to pay my child’s full season fee assessment as the rest of the team cannot be expected to pick up their share.

We have read the material provided and understand the time commitment involved in practice and competition. Waiver of Liability and Payment Schedules
The undersigned player and parent/guardian hereafter designated as member, agree to accept membership in the for the 2013 volleyball season. Member agrees that Timpview High School, Victory Volleyball Club owners, employees, and staff members shall not be liable for any damages arising from personal injuries sustained by the member and/or guest(s), on or about the premises of the said facilities or by participation in events with Victory Volleyball.
These agreements I have read, understood and agree to the above terms. This contract is not transferable, not cancelable by member.

Volleyball League Code of Conduct
Being a member of the UVBHS Volleyball Club requires a commitment to being the very best person I can be...both on and off the court. In order to ensure the safety and well being of the athletes of the UVFL and to best represent the program, players, parents, and the coaching staff, I hereby consent to abide by the following code of conduct:
1. I will show good sportsmanship toward opponents, teammates, officials, and spectators. I will treat other teams and their members with the same respect in which I would want them to treat the UVFL program and me.
2. I will refrain from any physical action that may endanger the safety and well being of an opponent, teammate, coach, spectator or official.
3. I will refrain from any verbal abuse that might detract from the integrity of the program or any individual.
4. I agree to respect the property of others (teammates, gym sites, hotels, etc...) and to pay for any damages that I am found responsible for.
5. I will maintain a respectable and responsible court demeanor at all times. I realize that I am part of a team and my personal goals and ambitions are second to the goals and ambitions of the team.
6. I agree not to be in possession of or use drugs, tobacco or alcohol while under the supervision of the UVFL. I also agree not to be in the presence of such substances or to be found equally at fault. (Supervision includes practice, team outings, tournaments, and trips from departure thru return).
7. I will support all other UVFL Volleyball teams and players. I appreciate their hard work in helping to make the league the best it can be.
8. I understand that I will not be allowed to participate in any matches without my
uniform. I also understand that I may not alter my uniform in any way (cut off
sleeves) until the conclusion of the tournament.
I understand that violation of this code of conduct will result in disciplinary action. (Match suspension, etc...) Violation of Article 6 will result in immediate dismissal from the program. There will be no financial reimbursement for violation of this policy, which results in dismissal. Parents will be notified regarding any violation that would result in more than a single match suspension.

Waiver and Release of Liability
I hereby give my daughter/son ______permission to participate in the league conducted by UVFL. I acknowledge that volleyball or any sporting event can cause injury, death or property damage. With a full understanding of the potential risks, I hereby assume all such risks of participation. Further, I (a) waive, release, and discharge from any and all claims or liabilities for death or personal injury or damages of any kind which arise from such participation, the following: UVFL, its coaches and staff, and owners of any facilities utilized by UVFL, (b) agree not to sue any of the parties previously listed, and (c) indemnify and hold harmless the persons or entities previously listed from any claims made or liabilities assessed against them as a result of my actions.
