WRITTEN HAZARD COMMUNICATION PROGRAM

I. INTRODUCTION
It is the desire and intention of Company Name to provide a safe and healthful working environment for all of our employees. In this regard, a Hazard Communication Program has been implemented to ensure that all workers whose jobs involve the handling of hazardous chemicals are not only fully aware of all of the hazardous chemicals involved, but that they are thoroughly trained to perform their job safely.
The Hazard Communication Coordinator for this program is ______. The responsibilities of the Hazard Communication Coordinator include coordinating chemical lists, hazard evaluation, MSDS acquisition; organizing training activities; and administering the program.
II.LABELING AND OTHER FORMS OF WARNING
A.At this unit, the Hazardous Communication Coordinator is responsible for ensuring the labeling of in-plant primary and secondary containers. Periodic plant reviews will be conducted to ensure that labels are upgraded in accordance with new toxilogical studies and previously unknown physical properties have been identified.
B.See Attachment Afor a copy of the labeling system used.
C.As new hazardous chemicals arrive for use at this unit, purchasing representatives will ensure that all containers of raw materials have been checked against the chemical lists to determine if they are properly labeled. The chemical name on the container must match the chemical identity on the SDS/MSDS to insure they are the same.
D.Manufacturers of containers with inadequate labels will be contacted and requested to update their product. Purchasing representatives are responsible for contacting vendors whose containers have inadequate labels and, when necessary, such vendors will be notified that unit representatives will look for product substitutes.
E.Secondary containers of chemicals which are used by more than one employee and last for more than one work shift have been labeled with information similar to that required for manufacturers' containers.
F.Portable Containers intended for immediate use need not be labeled if the hazardous chemical will be under the control of and used by the person who transfers it from a labeled container during the work shift in which it was transferred.
III.MATERIAL SAFETY DATA SHEETS
A.Purchasing Representative, ______,is responsible for obtaining and maintaining an up-to-date file of MSDS's for this site.
- The SDS/MSDSdata sheets are maintained in yellow binders labeled "MATERIAL SAFETY DATA SHEETS", and binders are kept at the following locations:
Each employee shall be informed of the SDS/MSDS binders and where they are kept. Employees shall have ready access to binders during every shift.
C.If an SDS/MSDS is not received with the order of a hazardous substance, purchasing shall immediately telephone the supplier and request a data sheet. The purchasing representative shall make documentation as to date and time of this phone call. If a data sheet is not received within one week, a letter will be sent by the purchasing representative to the supplier and a copy of this letter will be kept on file. If a data sheet is still not received within ten days, this site will cease use of the product and will find a substitute. Further orders should not be taken from this supplier.

D.SDS/MSDS shall be the primary source of information on substances. Safety data sheets which are similar can not be substituted in place of the originals.
IV.TRAINING
A.______will be responsible for implementation of the Hazard Communication Training Program. In addition, the management and supervisory personnel listed below will be responsible for specific training when needed:
B.Each training program will utilize a specific format depending on the subject matter. Since VCR and slide/tape equipment will be heavy utilized, training sessions will primarily follow a classroom format. In many cases generic programs will be reviewed on a VCR or on slides and the trainer or supervisor will then give a more specific description of the particular hazardous substances within that department and how to safely work with each product.
C.Elements of the training program shall consist of at least the following:
1.An overview of the contents and requirements contained in the hazard communication standard.
2.Chemicals present in employee work areas and departments.
3.Location and availability of the Company's written hazard communication program.

4.How to detect the presence or release of a hazardous chemical in the work area. (Including information pertaining to periodic monitoring methods, continuous monitoring devices, visual appearance or odor of hazardous chemicals when released, etc.)
5.Physical and health hazards of the chemicals in the work area.
6.Measures employees can take to protect themselves from these hazards, including specific procedures the Company has implemented to protect employees from exposure to hazardous chemicals, such as appropriate work practices, emergency procedures, and personal protective equipment, etc.
7.A review of the Company's hazard communication program, which will include an explanation of the Global Harmonized System (GHS) labeling system, how to read a safety data sheet, and how employees can obtain and use the appropriate hazard information.
D.New or relocated employees will be trained regarding hazard communication within FIVE days of starting in a new area or job. Employees will receive training immediately, and prior to the introduction of a new hazardous chemical to their job.
E.Documentation of employee training shall be done by use of attendance listings, training certificates, and/or by post training quizzes. Quizzes shall be reviewed, with answers given, after each training session.
V.LIST OF HAZARDOUS CHEMICALS
A.A chemical list has been compiled of all materials present at each unit and within various departments or key work areas. Chemical lists will be updated whenever there is a change in the process or supplier.
B.______is responsible for maintaining current chemical lists in key work areas at this facility. (See Attachment B).
C.Chemical lists will be updated on an as need basis.
D.Employees will not be allowed to bring hazardous chemicals, including consumer products, to work. All requests for chemical purchases must go through purchasing. The unit Hazard Communication Coordinator will be responsible for monitoring this practice.

VI.NON-ROUTINE HAZARDS AND EMERGENCIES
A.Non-routine jobs/situations and types of emergencies where exposure to hazardous chemicals might occur have been identified for various key areas of this facility. Written guidelines have been developed which describe special procedures and emergency plans for these situations have been developed and affected employees shall be trained in these special procedures and plans. This training shall be conducted by the supervisor in the affected area. (See Attachment C).
VII.LABELING OF PIPES
All pipes at this facility are labeled as to their contents. The labeling is as follows:

VIII.CHEMICALS IN UNLABELED PIPES
A.When work is to be performed in areas where chemicals are transferred through unlabeled pipes, the department supervisor will inform workers of:
1.The chemicals in the pipes.
2.The potential hazards.
3.Safety precautions and protection which should be taken.
IX.ON-SITE CONTRACTORS

______, will assure that contractors are informed of any potential chemical hazards in areas where they work prior to beginning the work. This will be done by meeting with the contractor's supervision and reviewing with them the hazardous chemicals present, their properties, and what to do in case of an emergency. Also, contractors will be required to inform the Company's Project Coordinator of any hazardous substances which they will be using in their work.
