Where: Prince Arthur’s Landing at Marina Park Pearl St.Entrance @ boat launch

Registration Social: Prince Arthur Hotel on Friday, March 28th, 2014 between 5:00 & 9:00 p.m.

Late Registration: -Prince Arthur Hotel on Saturday, March 29th from noon to 1:30 p.m.

Plunge Time: 2:00 - Adult Team and Individual Plunge

3:00 – Cool School Plunge

Awards & Social – Prince Arthur Hotel from 3:00 to 6:00

This year, the Law Enforcement Torch Run Polar Bear Plunge will raise money for Ontario Special Olympics to support the hardworking athletes who are involved.

Special Olympics Ontario provides individuals in our community and across Ontario with an intellectual disability the opportunity to participate in sport and recreation activities.

Our goal is to raise $40,000.00 at this event!

Once you register on line you may start your online donation campaign or download the pledge forms to get your friends and family to help you reach your fundraising goal. A waiver form must also be signed online or you can download the form and hand it in at the event.

A minimum of $100 registration fee (or pledges) is required to participate in the Plunge.

All high school aged participants are welcome to participate in the Cool Schools Category and require a $75.00 registration fee (or pledges).

Prizes for taking the PLUNGE!

$75 (Cool School only) or $100- Receive a Polar Plunge t-shirt

$300- Receive a Polar Plunge t-shirtand Polar Plunge Towel

$600– Receive a Polar Plunge t-shirt, a Polar Plunge towel and a Polar Plunge gym bag

$1,000– All Above incentive prizes a chance to win a pair tickets to anywhere WESTJET flys.

BestCostume-Organizers reserve the right to regulate what costumes can be worn in the water for safety concerns. Prizes will be awarded to the best adult and best cool school costume.

Top Fundraisers– Win two tickets to a Toronto Maple Leaf Game and a chance to meet Special Olympics Ambassador Jake Gardiner after the game. Accommodations will also be provided with a two night stay in a downtown Toronto hotel.

5th Annual Polar Bear Plunge Event Entry and Waiver Form

In consideration of participating in the Polar Bear Plunge, I represent that I understand the nature of the event and that I/or my minor child am qualified, in good health and in proper physical condition to participate in such an event. I acknowledge that if the event conditions are determined to be unsafe, I and/or my minor child will immediately discontinue participation in the event.

I fully understand the event involves risk of serious bodily injury, including permanent disability, paralysis and death, which may be cause by my own actions, or inactions, those of others participating In the event, the conditions In which the event takes place, or the negligence of the “releasees” names below, and that there may be other risks either not know n to me or not readily foreseeable at this time; and I fully assume and except all such risks and all responsibility for losses, costs, damages I incur as a result of my participation in this event.

I hereby release, discharge, and covenant not to sue the Thunder Bay Port Authority, the City of Thunder Bay, Lakehead Public School, Thunder Bay District Catholic School Board,the Law Enforcement Torch Run, Special Olympics Ontario, their respective administrators, directors, agents, officers, volunteers, and employees, emergency personnel, other participants, any sponsors, advertisers, and, if applicable, owners, and lessors of premises on which the Activity takes place, (each considered on the “RELEASEE’s herein) from all liabilities, claims, demands, losses, or damages on my account caused or alleged to be caused in whole or in part by the negligence of the “releasees” or otherwise, including negligent rescue operations; and I further agree that if, despite the release, waiver or liability, and assumption of risk I, or anyone on my an/or my minor child’s behalf, make a claim against any of the Releasee, I will indemnify, save, and hold harmless each of the releasees from any such loss, liability, damage, of cost which each of the releasees from any such loss, liability, damage, of cost which may incur as the result of such claim.

I have read this release and waiver of liability, assumption of risk, and indemnity and fully understand it. All participants must be 12 years of age or older.
