POSITION DESCRIPTION

FSET Case Manager

GENERAL FUNCTION

Perform intensive case management services to assess the needs, strengths, and barriers of FSET participants. Work collaboratively with participants to develop a plan that works to assist them in reaching their employment goal.

REPORTS TO

FSET Program Manager

SUPERVISES

No supervisory responsibility.

RESPONSIBILITIES

A. ESSENTIAL FUNCTIONS

1. Provides case management services including locating, managing, coordinating and monitoring all services and informal community supports needed by FSET participants. Services include assessment, case planning, monitoring, review, advocacy and referral.

2. Case management activities provided through the team approach are:

  1. Summary and analysis of subjective and objective data through individualized assessment.
  2. Planning of appropriate activities to address the assessment results.
  3. Determining the intervention strategies that will address the identified needs and goals.
  4. Implementation of the plan and intervention strategies.
  5. Ongoing progress monitoring and evaluation of employability plans.

3. Ability to motivate others towards achieving their personal goals.

4. Ability to work independently with strong sense of focus, task-oriented, nonjudgmental, open personal qualities, clear sense of boundaries.

5. Ability to work in a variety of settings with culturally-diverse families andcommunities with the ability to be culturally sensitive and appropriate.

6. Provide holistic and comprehensive case management services to all participants. Including: intake assessment, benefit assessment, goal setting, long-term case plan development, bi-weekly case plan development, progress monitoring, advocacy and referral.

7. Excellent written and verbal communication skills, ability to establish rapport with participants and Income Maintenance staff.

8. A strong sense of and respect for confidentiality involving both clients and fellowemployees.

9. Develop relationships with local employers to understand in demand jobs and assist program participants in gaining employment.

10. Assist participants in completing job applications, writing a resume, and documenting work history.

11. Developing worksite and workfare experience sites, and monitoring participation in both programs.

12. Maintaining accurate and detailed accounts of all participation in the ABAWD and FSET Programs.

13. Develop relationships with community service providers to ensure duplication of services does not occur. Work collaboratively with providers to provide successful outcomes for participants.

B. OTHER JOB DUTIES

1.Promote self-sufficiency and upward movement in the work world through training, education and counseling to reduce participant turnover and failure.

2.Promote skills training and provide reinforcement to reduce work program sanctions and loss of benefits.

3.Promote strategies that will reduce the recidivism rate FSET participants losing Food Share benefits.

4.Any other duties as may be assigned.

EXPERIENCE, TRAINING, QUALIFICATIONS

High School diploma as well as a Bachelor’s Degree in Behavioral Science or Human Services field. Knowledge of human behavior, motivation, crisis intervention, family and group dynamics, money management, and organizational skills are necessary. Interviewing and assessment skills are required. Participation in State or Agency training is required. Basic everyday living skills are needed, as is the ability to understand and follow oral and written directions. Reading, writing, adding, and subtracting skills are needed. Must have access to transportation to travel to various work sites.

Basic office equipment used: copy machine, personal computer and printing equipment, FAX machine, calculator, and telephone.

PHYSICAL REQUIREMENTS OF THE ESSENTIAL FUNCTIONS

Seventy-five percent (75%) of the time is spent sitting, feeling, hearing, talking, and using near vision. Fifty percent (50%) of the time is spent bending/twisting, reaching, feeling, and low lifting and carrying. Twenty-five percent (25%) of the time is spent low fingering. Standing, walking, balancing, and low pushing/pulling comprise ten percent (10%) of the time. Required in unusual or non-routine situations, stooping, kneeling, crouching, crawling, running, grappling, climbing (using legs and feet), climbing (using legs and arms for support), using far vision, and medium lifting and carrying.
