Items to Bring

Each day during the summer your child will be very busy with lots of activities. It is important to us that your child is comfortable while they are here. Below is a list of items your child should have on hand with them while they are in attendance. Please label all of the items with your child’s name.

  • Bathing Suit
  • Towel
  • Sun Screen with form
  • Hat
  • Water shoes
  • 2 changes of clothes
  • Sweatshirt
  • Water bottle
  • Lunch box with an ice pack
  • A good book to read
  • A bag to store their items (the Aldi’s recyclable bags are durable and work very well)
  1. Every day will be spent engaging in water play via a sprinkler on the playground. Please make sure your child has the appropriate gear to do so.
  2. After lunch the school agers will have at the very least one half hour for quiet reading time, this is a great opportunity for your child to complete their summer reading list. Please make sure your child has reading material that is of interest to them available.

GENERAL RELEASE AND HOLD HARMLESS AGREEMENT – MINOR

I, ______, am the parent or legal guardian of ______(the “minor”), who desires to participate in various programs, events or activities (hereinafter collectively referred to as the “Activities”) operated or sponsored by the Education Center of the New Milford United Methodist Church (hereinafter collectively referred to as the ”Education Center/UMC”).

I understand and acknowledge that the Education Center/UMC will not allow the minor to participate in the Activities without releasing and holding the Education Center/UMC harmless from any liability arising out of participation in the Activities. I have investigated the risks involved in the minor’s participation in the Activities and fully understand and assume such risks on his or her behalf.

I REQUEST THAT THE EDUCATION CENTER/UMC ALLOW THE MINOR TO PARTICIPATE IN THE ACTIVITIES, AND IN CONSIDERATION THEREOF AGREE HEREBY TO RELEASE AND FOREVER DISCHARGE THE EDUCATION CENTER/UMC, ITS OFFICERS AND DIRECTORS, AND ITS EMPLOYEES, AGENTS, AND ANY PARTIES VOLUNTEERING ON BEHALF OF THE EDUCATION CENTER/UMC FROM ALL ACTIONS, CAUSES OF ACTION, INJURIES, CLAIMS, DAMAGES, COSTS OR EXPENSES OF ANY KIND GROWING OUT OF OR RELATED TO ANY SUCH ACTIVITIES IN WHICH THE MINOR PARTICIPATES. I UNDERSTAND THAT THIS IS A FULL AND COMPLETE RELEASE OF ALL INJURIES AND DAMAGES WHICH I OR THE MINOR MAY SUSTAIN AS A RESULT OF HIS OR HER PARTICIPATION IN ANY OF THE ACTIVITIES, REGARDLESS OF THE SPECIFIC CAUSE THEREOF.

I further acknowledge and agree that I have given my consent for the minor to participate in the Activities and to remain in the custody of the Education Center’s/UMC representatives while participating in the Activities.

This agreement is binding on the minor’s heirs, successors, and personal representatives.

MEDICAL TREATMENT AUTHORIZATION

In the event the minor suffers an injury or condition during his or her participation in the Activities, including transportation to and from the Activity, which may endanger his or her life, cause disfigurement, physical impairment, or undue discomfort, I hereby authorize the staff of the Education Center/UMC to:
