I received your request for a personal meeting at the Study Skills Center. I would like to explain and clarify the following:
a.Target Population:The service is offered only to students who have been recognized by the Dean of Students as students with learning disabilities /ADD/ADHD.
b.Center Activities: The services will be given during individual 50 minute meetings
c.Payment: The service is provided at a fee of NIS 50 per individual. . This is a symbolic fee for the service. All fees paid by the students will be invested into the Center to improve and extend the services it provides the students.
d.Effect Payment:Payment must be effected before your meeting to Ms. Alice Mazkeret/Sigal Israeli of the IDC Finance Department (Near the Tuition's Office).

You have to keep the receipt and bring it with you to every meeting at the study skill center.
e.Commitment: Registration for a personal meeting constitutes a commitment to attend the meeting. The hours allotted to the StudySkillsCenter are few and each and every hour is important!
f.Cancellation of Meeting: If you should not be able to attend the personal meeting (or the small group meeting), you are kindly requested to notify the Center in writing (by email) addressed to Ms. Shirley Menda and to the teacher no later than forty eight (48) hours before the meeting. Should you not notify the Center of cancellation of the meeting you shall be charged for the fee for the meeting.
Note: Any student who does not attend more than one meeting and did not notify the Center of their cancellation in advance – shall not be able to continue activities at the Center without clarification with the Center's Director.

The Dean of Students acknowledges that I have learning disabilities and I have been granted special conditions during exams.
I agree that the teacher at the StudySkillsCenter can review my diagnostic report submitted to the Dean of Students, from time to time as needed.

I am aware that sending this email is considered undertaking to attend the meeting that will be set for me. Should I not be able to attend, I know that I must send notification of cancellation of the meeting by email to Ms. Shirley Menda and to the teacher at least 48 hours prior to the scheduled meeting.
I know that should I not send notification of cancellation at least 48 hours before the meeting I will be charged the fee for the meeting.
