The Parents Or Guardian of Students Enrolled in the Pittsburgh Chinese School Must Sign

The parents or Guardian of students enrolled in the Pittsburgh Chinese School must sign the Registration Form to acknowledge and agree to this document.

As the parent or Guardian of the student enrolled in the Pittsburgh Chinese School, I have voluntarily enrolled in the Pittsburgh Chinese School (hereafter “PCS”) for the courses listed above to be conducted on the premises of Allderdice High School and of any other locations that PCS has contracted or arranged for use. I agree to hold PCS, its Board of Directors, administrators, employees, instructors,and any other representativesrelated to PCS harmless for any injuries, damages, and accidents suffered as a result thereof. I hereby acknowledge that participation in theeducation program at PCS including but not limited to the above courses and any other related activities is at my own risk and that PCS, its Board of Directors, administrators, employees, instructors, and any other representatives related to PCS are not responsible for any injuries, damages, and accidents incurred while I am participating in the entire program activities.

RELEASE

In consideration of my agreement to participate in the above mentioned program, and intending to be legally bound hereby, I for myself, my guardians, my heirs,my legal representatives, and any other of my representatives agree to forever discharge PCS, its Board of Directors, administrators, employees, instructors, legal representatives, any other representatives related to PCS, and their successors, of and from any and all claims, demands, rights and courses of action of whatever kind and nature, arising from and by reason of any and all knownand unknown, foreseen and unforeseen personal injuries, damages, and accidents (including death) that have been and that may be sustained by me as a result of, or in any way related to, my participation in the above mentioned program and activities.

INDEMNIFICATION

As additional consideration for the agreement of PCS to allow me to participate in the said program, and intending to be legally bound hereby, I agree to indemnify PCS, its Board of Directors, administrators, employees, instructors, legal representatives, any other representatives related to PCS, and their successors, from any and all loss, claims, demands, and actions at law or in equity that may hereafter at any time be brought by me or anyone acting on my behalf, for the purpose of enforcing a claim for damage because of any injury, damage, and accident (including death) sustained by me as a result of, or in any way related to my participation,in the above mentioned program.
