Review the following information about The Lynda DeBerry Legacy Scholarship,

and apply if you are eligible.

Scholarship Components:

All of the following must be accomplished before your application will be considered.

The April 1st Checklist: All of the following is to be submitted on or before April 1st.

  1. ____ Completed application (see page 2)
  1. ____ Write a five-paragraph, personal statement (500-1500 words) about one of the listed writing prompts (see page three for further details).
  1. ____ Include proof of cumulative GPA (see guidance counselor for printouts).
  1. ____ Attendance records for your junior and senior years (see guidance counselor for printouts).
  1. ____ Disciplinary records for your junior and senior years (see guidance counselor for printouts).
  1. ____ Submit a signed, original copy of a letter of recommendation from a MHS teacher, librarian, principal, counselor or paraprofessional.
  1. ____ Submit a signed, original copy of a letter of recommendation from a community source. This cannot be a letter written by a family member.

Recipient Information: If you are the recipient of the Lynda DeBerry Legacy Scholarship, you also must provide the following by the first week of June.

1.  Proof of admittance to a university, college, or technical school by June 30

2.  Proof of registration for classes as a full-time student (12 semester hours or more) by June 30

3.  Legal form of photo identification

Who is Eligible to Apply?

·  Millennium High School seniors who have been accepted to a university, college, or technical school.

AND

·  Millennium High School seniors who have exhibited high moral character and citizenship.

Deadline:

Application deadline is April 1st for consideration for the current academic year. Applications must be received by or postmarked by April 1, to be considered.

Congratulations, Millennium High School senior. Your determination has gotten you this far. In addition to your personal work ethic, I am certain that you know of others who have helped you on this educational odyssey. People like your parents/guardians, teachers, counselors, principals and teacher’s aides have undoubtedly been role models. One such role model was Millennium High School’s very own librarian Ms. Lynda DeBerry. She was a wonderful educator, Mother, Grandmother, Agua Fria High School teacher, Millennium librarian, and friend. All of her children graduated from our fine district and so have two of her grandchildren. Lynda was a Child Development teacher and often team-taught Children’s Literature. She loved education, and she earned more than one Masters Degree. Most of all, she loved helping students to love books. In memory of this beautiful woman, certified and classified employees of Millennium High School have founded The Ms. Lynda DeBerry Legacy Scholarship. They have made this scholarship possible. With this in mind, please compose a five-paragraph, personal statement (500-1500 words) about one of the following writing prompts:
