THE KIWANIS CLUB OF TORONTO FOUNDATION SCHOLARSHIP
2015 APPLICATION FORM

To be eligible for The Kiwanis Club of Toronto Foundation (KCTF) Scholarship the applicant must:

  1. Be “Toronto-based”[residing and/or studying in Toronto (the amalgamated former Metropolitan Toronto)] AND show need;
  2. Have been registered in full-time post-secondary studies for the 2014-2015 academic year;
    [please note: an exception to this requirement may be made for applicants who are currently secondary studentswith developmental disabilities with Individual Education Plans (IEP)and who are planning to enroll for one year vocational programs for 2015-2016; these applicants need to make this circumstance clear in their application, especially section e)];
  3. Intend to register forpost-secondary studies, in the 2015-2016academic year at the 2nd year level or higher;
  4. Have been24 years of age or younger as of January 1, 2015;
  5. Have received no more than one (1) previous scholarship under this KCTF program.

To be considered for this scholarship the applicant must:

  1. Meet all of the above-noted eligibility criteria;
  2. Complete, print and sign this application;
  3. Submit the completed application by regular mail or fax or email (email may ONLY be used if the applicant is able to scan in supporting documentation and signature) along with supporting materials to the KCTF office (address at end of application) BY the application deadline of Friday, June 12, 2015.

In order to receive a scholarship support from The Kiwanis Club of Toronto Foundation (KCTF) as awarded recipients must:

  1. Be enrolled as a full-time student in2nd Year or higher level in university or college or registered apprenticeship for the academic year 2014-2015;
  2. Provide a copy of their 2014-2015fee statement to KCTF.

g)Please identify the educational institution for your most recent AVAILABLE statement of marks; in many cases this will be mid-year results, 2014-2015 post-secondary transcripts or for students now at the first year level, their final report card for high school. Submit this marks statement along with your application or when they become available.
If you do not have your final marks statement by the June 1st deadline, please submit your application and then send your final marks for this academic year to us as soon as they become available.
h)Please supply the names of two (2) individuals who have agreed to write letters of reference.
It is permissible to have ONE (1) ONLY of your two (2) references be from a member of the Kiwanis Club of Toronto, or other Kiwanis Club. NOTE:in cases where a referee is a Kiwanian, he/she must identify him/herself explicitly as a Kiwanian and specify their club affiliation.
Letters may be attached to the application and do not need to be sealed; should the referee wish to send the reference letter directly to The KCTF, it is the responsibility of the applicant to ensure that it arrives by the application deadline, June 12th.
A Reference letter may be sent by email a) if the referee’s email is listed below, b) the letter originates from the email address listed, c) the applicant’s full name appears in the subject line, d) the letter is sent “in-line” and e) the email is sent to The KCTF at .

YOUR SCHOLARSHIP APPLICATION WILL BE EVALUATED BASED ON YOUR COMPLETED APPLICATION AND SUPPORTING DOCUMENTATION. MAKE SURE THAT YOUR APPLICATION IS COMPLETE.

The KCTF may deem any applicant or recipient ineligible for an award if their contact information is invalid and/or they do not respond promptly at any time during the awards process.
The undersigned applicant(or if under 18, the applicant’s parent/guardian on their behalf) hereby applies for a scholarship from The Kiwanis Club of Toronto Foundation (The KCTF). I hereby give permission for The KCTF Scholarship Committee (“the Committee”) to review all information and documentation provided in support of my application and to request further documentation from me. If I am selected for an award, I understand that I may be invited to participate in promotional activities by The KCTF. Further, if I am selected for an award, I agree that The KCTF may release my name to its Scholarship Partner organizations and to allow my name and picture to appear in media materials to assist in the promotion of The KCTF Scholarship program.
