Instructions
The High School Allotment Advisory Group (Advisory Group), established in compliance with TEC 61.1098, was charged with making recommendations to the Commissioner of Education regarding standards for selecting school districts and campuses with exceptional high school completion and success and college readiness programs implemented with high school allotment funds.
The Advisory Group recommended that districts and campuses be recognized for exceptional use of their High School Allotment in four categories based on select criteria.
To be considered for recognition, districts must complete this form and submit it to TEA by January 29, 2009.

(To be considered innovative, a program must go beyond the normal operations and services provided by the school/district; must have been developed and implemented within the 2007-2008 school year; and, must have preliminary evidence indicating an impact upon college readiness, graduation rates, or successful transitions.)
Selection:
I. Please provide a brief description of the Allotment program or activities for which you are nominating your district or campus for Exceptional Use of the 2007-2008 High School Allotment Fund. The description must address the following selection criteria, as appropriate:
· Scope – number and percent of students participating and/or impacted.
· Students targeted – extent to which specific groups of students are targeted for services, e.g. high-risk or high-need students.
· Use of data – extent to which the Allotment program or activities are designed to respond to needs identified through data analysis.
· Monitoring – extent to which data are used to make corrections and improvement throughout the course of program implementation.
II. Describe the evidence or early signs of success for your district or campus’ Allotment program or activities that you are nominating for Exceptional Use of the 2007-2008 High School Allotment Fund. The description must address the following selection criteria, as appropriate:
· Student performance – degree of improvement in student performance.
· Disparity in performance among student groups – extent to which gap in performance among student groups - defined by ethnicity, gender, economic status and/or native language – is reduced.
· Evaluation data– type of data used to measure impact.
