Below you will find links to all of the necessary health forms you need to submit as a new student at Lehigh.

Be sure to read each document carefully and complete all forms with black ink. Send the original forms to:

All incoming students must complete the following steps tobe compliant with the requirements of the Health & Wellness Center by January 3rd:

  • Immunization Record- This form must be signed by your health care provider to verify that you have received the required immunizations.Students must enter dates online through Patient Portal at Log onto patient portal using your Lehigh ID and password, go to the immunization tab, enter, then to the enter dates tab: If you are missing any of the required immunizations please contact your physician’s office to make an appointment to receive them as soon as possible.
  • HealthForm: It is required for all students. Make sure you answer all of the questions, including the “”Yes/No “section. The second page of the Health Form is a physical Exam form to be completed by your physician. The physical exam is recommended for all students but required for Varsity Athletes.
  • TB screening questionnaire: Werequire all new students to answer a few simple questions regarding their risk for Tuberculosis (TB). These screening questions will identify students for required testing on campus. While on Patient Portal, click on the box that says: Appointment Scheduling and Patient Portal”. Then, click on the “forms ‘tab at the top. Answer the 5 questions and hit submit! Do not forget to complete the TB Screen.*Failure to comply may result in blocked registration.
  • Meningitis Compliance Form– must be completed in order for you to receive your room key from Residential Services, per Pennsylvania state law. Please read the meningitis information document before completing the form.

Varsity athletes (recruited or planning to walk on) must also provide copies of:

  • A physical examination within six months of participation at Lehigh
  • Results of Sickle Cell Trait testing or a completed Sickle Cell Trait Testing Waiver form

Allergy Information: Students who wish to receive allergy or other medication injections at the Health & Wellness Center can find detailed information about our policies and procedures on our website. Please print out this information and bring it to your allergist or other physician.

University Health Insurance Plan: The bursar's office will send you information on the university-sponsored health insurance plan, University Health Plans, designed to complement the services provided to students by the Health & Wellness Center. Information about this insurance plan can be found at We strongly recommend consideration of the plan even if you have other health insurance. It is particularly important for you to understand the possible limitations of your health insurance while you are attending college, including simple services such as lab tests and x-rays. Please know if you must utilize a specific lab; Quest, LabCorp, and Health Network have local branches. We recommend consulting your physician or insurance carrier before waiving the university health insurance. Remember to bring your insurance card with you to campus or a picture of the card on your smartphone.
