Gift Aid Form
The Government’s Gift Aid scheme means charities can reclaim tax on all gifts. All you have to do is fill in this form. It will increase any further donations you make to CEDE Foundation by a significant amount, at no extra cost to yourself, providing you are a UK tax payer.

Please read the following carefully
- This declaration covers this donation and any future donation you may make to CEDE Foundation but please:
- Notify CEDE Foundation if you change your name or address while the declaration is still in force
- You can cancel the declaration at any time by notifying the charity – it will then not apply
to donations you make on or after the date of the cancellation or such later date as you specify.
- You must pay an amount of income tax and/or Capital Gains Tax for each tax year (6 April one year to 5 April the next) that is at least equal to the tax that CEDE Foundation reclaims on your donations in the appropriate tax year.
- If in the future your circumstances change and you no longer pay tax on your income and capital gains equal to the tax CEDE Foundation reclaims, you can cancel your declaration (see note 1).
- Tax claimed by CEDE Foundation
a CEDE Foundation will reclaim 28p of tax on every £1 you gave up to 5 April 2008.
b CEDE Foundation will reclaim 25p of tax on every £1 you give on or after 6 April 2008.
c The Government will pay to CEDE Foundation an additional 3p on every £1 you give between 6 April 2008 and 5 April 2011. This transitional relief for the charity does not affect your personal tax position.
