METHODOLOGY:
The following assessment report is the result of an in-depth assessment process that included all of the following:
  • Review of Positive Achievement Change Tool (PACT) screening instrument
  • Review of the offender’s legal history
  • A structured interview with the offender (SAMH-2)
  • Consultation with the current Juvenile Probation Officer
  • Conversation with offender’s parent/guardian
  • A 5 panel urinalysis

LEGAL HISTORY:
  • 11/19/2017 – ASSAULT WITH INTENT TO COMMIT A FELONY (F3)
  • 11/19/2017 – RESIST OFFICER – OBSTRUCT WO VIOLENCE (M1)

EDUCATIONAL/VOCATIONAL HISTORY:
Justin is attending the 9thgrade at Atlantic High School. He reported thathe likes school, but he did not know his GPA. Justin’s grandmother reported that Justin is not doing well academically and is failing the 9th grade.Justin was expelled from elementary school on one occasion and suspended more than 10 times as a result of bad behaviors in school. Justin reportedly enjoys being the class clown and getting everyone’s attention. He denied that he has ever been retained. Justin expressed a desire to obtain his GED and to later pursue a career as a football player and/or an entrepreneur.Justin denied any history of employment and or ever having received any formal vocational training.
FAMILY HISTORY/ABUSE HISTORY:
Justin resides with his grandmother, grandfather and his younger sister.He expressed that he has a positive relationship with his family. He was raised by his biological mother until she passed away when he was 5 years of age. Justin reported that he has never seen or had a relationship with his biological father. No one in the home was reported to have been arrested, use substances or have any mental health conditions. Justin denied ever having been the victim and or perpetrator of physical, sexual, emotional abuse. He denied that he has ever ran away from home or that he has ever been kicked out of his home. Justinhas a curfew of 5:00 p.m., and reported the heis responsible for cleaning the bathroom, kitchen, living room and his bedroom.He shared that heis not disciplined in his home by either his grandmother or grandfather when he gets into trouble.
SOCIAL HISTORY:
Justin described himself as a quiet, cool and chilled individual who enjoys keeping to himself, yet enjoys engaging in outdoor activities, such as, fishing and playing football with his friends. He reported that he has a lot of friends who are both good and bad influences on him. Justin’s grandmother explained that Justin no longer associates with the friends whom he was arrested with. He denied that his friends/acquaintances engage in using drugs and/or alcohol.He denied ever having been a member of a gang.
SUBSTANCE ABUSE HISTORY:
Client was assessed for use of the following substances:

URINALYSIS RESULTS:
Justin was administered a 5-panel drug urinalysis at the time of the assessment and he tested negative for all substances:Cannabinoids (THC), Cocaine (COC), Opiates (POI), Methamphetamines (MAMP) and Benzodiazepines (BZO).
PHYSICAL HEALTH:
Justin reported that he is in good health. He was last seen by his physician February 2018 to be reevaluated and placed on Vyvanse and Clonidine for his diagnosis of Attention-Deficit/Hyperactivity Disorder (ADHD). He denied that he suffers from any physical or chronic health conditions or that he takes any other medications at this time.
MENTAL STATUS EXAM/PREVIOUS PLACEMENT:
Justin was oriented to person, place, time and situation. Hisinsight, judgment and appearance were within normal limits. He was very alert and cooperativeduring the assessment. He appeared to be guarded in his disposition, but he did not react in an adverse way to the questions asked of him. He denied any history of mental health regarding himself or anyone in his family. He denied any history of psychiatric services or treatment, and or ever having been prescribed psychotropic medication other than his medications for his ADHD diagnosis. Justin denied any past/present, suicidal/homicidal, ideations/attempts and or plans. He denied any history of hallucinations, delusions,and or obsessions. He denied ever having hurt animals or ever having set fires intentionally.
SUMMARY AND RECOMMENDATIONS:
Justin is a 15-year-old African-American male who was court-ordered to complete a TASC assessment. He is a first-time offender and was arrested on 11/19/2017 for assault with intent to commit a felony and resisting officer without violence. Justin reported that he was arrested as a result of throwing rocks and bottles at vehicles and running from law enforcement.
Justin resides with his grandparents and his younger sister. He has been residing with his grandparents since the age of 5 as a result of his biological mother passing away. He reported that he was raised by his mother up and till her passing, but expressed that has never met his biological father. He reported that he has a positive relationship with his family. Justin attends regular classes at Atlantic High School and is in the 9th grade. He expressed that he enjoys school; however, it was reported that he is not performing well academically and is currently failing the 9th grade.
Justin reported that he was diagnosed with Attention-Deficit/Hyperactivity Disorder (ADHD) and placed on medication. He explained that he was off of his medication for approximately a year before being placed on Vyvanse 70mg and Clonidine 2mg by his primary care physician in February 2018. His grandmother expressed that Justin’s behaviors since being placed on his medication has changed as he no longer yells and disrespects her and she has not received any phone calls from his school concerning any inappropriate behaviors. Justin denied any history of psychiatric services. He denied ever having participated in mental health and or substance abuse counseling/treatment. Justin’s grandmother reported that Justin did engage in some grief counseling while attending Pine Grove Elementary, but Justin no longer wished to engage in counseling. He denied any past/present, suicidal/homicidal, ideations/attempt and or plans. He denied ever having set fires and or ever having hurt animals on purpose.
Justin denied the use/abuse of any substances. He was administered a 5-panel drug urinalysis at the time of the assessment and he tested negative for all substances:Cannabinoids (THC), Cocaine (COC), Opiates (POI), Methamphetamines (MAMP) and Benzodiazepines (BZO).
Based on the information gathered at the time of the assessment, Justin is to comply with the obligations of his probation and to take his medications as prescribed. No other recommendations are being made at this time.
