Having received the Lord Jesus Christ as my Savior, and having been baptized, in the name of the Father, and of the Son, and of the Holy Spirit, and being in agreement with the vision, and Statement of Beliefs of Pleasant Heights Baptist Church, and having been led by the Holy Spirit to unite with this congregation, I do now, in the presence of God, most solemnly and joyfully enter into this covenant relationship with one another as one body in Christ as follows:
