THA Team Managers:

The following ** forms must be given to me to roster your team. Forms that DO NOT have ** in front of them DO NOT need to be turned into me. You must keep these forms in a binder and taken with you to all games and scrimmages.

Keep in mind, your team cannot scrimmage or play a game until the team has an official roster.

PLAYER

  • **Waiver of Liability/IMR (bar coded document generated when player registers with usahockey.com)
  • **Copy of Birth Certificate (ONLY IF PLAYER IS NEW TO THE THA AND DID NOT PLAY IN SPRING)
  • ** Concussion Statement Form – Parent/Participants Written Statement
  • THA Registration Confirmation (Players must register with the THA before they are rostered)
  • Consent to Treat
  • Players Code of Conduct Form

COACH

  • **Waiver of Liability/IMR (bar coded document generated when coach registers with usahockey.com)
  • **Background Check Form/Coaching Ethics Agreement (hand-signed coaching ethics forms will not be

accepted)

  • ** Concussion Statement Form – Coach’s Written Statement
  • THA Registration Confirmation (Coaches must register with the THA before they are rostered)
  • Safesport Training (Must be taken by Dec. 31, Good for 2 Years, Free of Charge)
  • USA Hockey Age-Specific Module (Must be taken by Dec. 31 if not taken last year for specific age group)
  • Copy of Coaches Card
  • Consent to Treat

MANAGER

  • **Waiver of Liability/IMR (generated when manager registers with usahockey.com)
  • **Background Check
  • THA Registration Confirmation (Managers must register with the THA before they are rostered)
  • Consent to Treat

PARENT

  • S.T.A.R Form
  • Background Check
  • THA Parent Acknowledgement Form

INSTRUCTIONS

*Put each of the ** forms in alphabetical order (players 1st, coaches 2nd, managers 3rd). Do not give me any documents that do not have a ** in front of them.

*Place in a large envelope or folder with your head coach, manager’s name, email and phone, and team name on the front. DO NOT STAPLE any of the forms together or put them in plastic sheet protectors.

*Put the envelope/folder in the “THA INBOUND” box on my porch and email me to let you know you have done so.

*Once your official roster is finalized, I will email it to you and you can pick up the ** paperwork from the “THA OUTBOUND” box on my porch.

IMPORTANT

*DO NOT email any required documents to me. Any and all documents must be provided in hard copy form.

*DO NOT turn in any original birth certificates. All birth certificates must be copies.
