Team Leader- PIVOT Community Support (Pembrokeshire West Wales)

Location – Pembrokeshire, West Wales

Hours – 35 hrs per week

Salary - £18,805.00 per annum plus £500 per annum unsocial hours allowance

Package:Fixed term to March 2019

The PIVOT service currently offers low level practical support to prevent unscheduled admission to hospital and supports individuals being discharged home. We are looking for a positive, energetic and enthusiastic person to oversee the strategic delivery and day to day management of the PIVOT service across Pembrokeshire, reporting to the Independent Living (IL) Service Manager.

In addition to line managing asmall staff and volunteerteam operating across Pembrokeshire the post holder will work with the IL Service Manager to ensure that the contract and reporting requirements of commissioners are met.

The post holder will drive continued development of the service, and in addition to the above will have responsibility for:

Relationship management with Royal Voluntary Service (RVS)/Pembrokeshire Association of Community Transport Organisations, Care & Repair (C&R), Pembrokeshire Association of Voluntary Services and funders

Expansion and future sustainability of PIVOT

Driving forward continuous improvement of service delivery

Disseminating learning from the service with strategic stakeholders in Pembrokeshire

PIVOT is a partnership project between the British Red Cross and the RVS, C&R, PAVS Hywel Dda Health Board and Pembrokeshire Unitary Authority. This post is initially fixed term until 31st March 2019 but will be ongoing subject to the renewal of funding. This post will be line-managed by the British Red Cross but the post holder will work closely within the collaborative partnership.

You will receive regular briefings, team meetings, supervisions, training and support. We provide a structured induction with all the in service training you will require.

The staff and volunteer team for which you will have overall strategic responsibility operates across Pembrokeshire unitary authority. The team itself comprises of 5 staff.

Building on the success of the existing PIVOT service this team, will provide a low level practical support to prevent unscheduled admission to hospital and supports individuals being discharged home.

You will be committed to achieving excellent standards of service delivery for our users and will lead your team in a supportive, caring and professional manner to enable them to achieve high quality person centred support at all times.

You will be accustomed to project managing services provided over a wide geographical area (Pembrokeshire)have experience of distance managing staff and have a proven track record in successfully delivering on contracted services.

You will have experience in all aspects of service development including writing reports, the evaluation and monitoring of services, budget managementand ensuring robust quality assurance systems are in place across the services you manage and you will have excellent communication and interpersonal skills along with good ICT skills.
