Teacher Instructions

For Interim IEPs

1st Complete Child ID > give to PAC rep. with copy of any and all IEP &/or Evaluation information; if no information is available, give name and phone/address of previous school

2nd Student Profile > must be created on SER by Donna Spell prior to teacher writing the Interim IEP

3rd Call and set up meeting ASAP > send Prior Notice and schedule immediately!

(Document ~10 day notice waived due to Interim IEP/Placement)

4th IEP conference w/ team, as usual > administrator, sp ed. teacher, reg. ed. teacher, parent, student, related service provider (if applicable)

Interim IEP:

5th IEP pages as usual, except for:

·  General Student Information page:

a. Click the drop down menu for “Type of IEP” > Select Regular - Interim

b. Exceptionality > will be grayed out

c. Evaluation date > will be grayed out

d. Summarize each section utilizing the information available (from out of parish, out of state, parent, etc.) or make comment that information is not available

e. Reevaluation comment > During the interim process, ______will be evaluated within 60 days. If he/she meets criteria as a student with a disability, then the IEP team will reconvene and write a COMPLETE Initial IEP to determine proper placement and appropriate programming. If he/she does not meet criteria, then, sp. ed. services will be discontinued.

·  Instructional page: 1-2 academics/cognitive goals & social, motor, etc. if needed for a 60 day period

·  Program/Services page:

a. Duration > 60 days (not 1 year)

b. Sp. Ed. instructional minutes should be the same or at least within the same “block” of time as indicated on information received

c. Related Services - if any noted on information received, include at least minimum time and write a statement in the Comment Section > Related Services will be delivered for diagnostic purposes during the interim process

·  Accommodation page: consult w/ the team; make sure to include reg. ed. teacher and parent

·  Placement/Least Restrictive Environment page: same placement as in previous parish or state until evaluation is complete, then the IEP team will determine appropriate placement at the reevaluation/IEP team meeting.

6th Parent signature also needed on:

a. Parent Permission for Evaluation

b. Release of Confidential Information

7th Send copy of IEP, Parent Permission, and Release of Confidential Information to: PAC – Attention > Donna Spell – Interim IEP

8th Start screening packet with SBLC ~ immediately ~ Evaluation timeline begins the day the Interim IEP is signed.
