PRE-INSPECTION SUMMARY

The Target Store at7th and Figaro,Los Angeles Central, CA was inspected by Thomas Branham of Elevator Consulting Services, Inc. on February3, 2012. My QEI number is 1339 and am certified by NAESA. NAESA is the National Association of Elevator Safety Authorities. We have identified corrective items that in our opinion need to be addressed by the Elevator Contractor. They are free to offer an explanation or dispute any individual item within the report but we can offer a reasonable reason for each and every negative evaluation. They are evaluated on and for the ASME 17.2 Code for Inspectors Manual and industry standards for acceptable installation and maintenance procedures.

FREIGHT ELEVATORS RE9, RE10

The following minimum items need corrective action taken. Documentation on completion is required.

  1. Install the “No Passenger” signage required by ASME 17.1 in each elevator
  1. There is no outside phone lines going to the elevators, the elevator cab phone does go to the security desk. Although this meets Code as it does go to a 24 hour call center, Target FMOSS has no way to track entrapments unless originated by the LOD through Target. Target’s call as to let it go because it meets code or implement a process where Brookfield Security contacts Alert One when issues arrive. Target’s main intent is to forward all calls to Alert One for quality assurance purposes.
  1. Provide “Elevator Equipment Room” signage on the machine room door. Turner wanted to paint the door first, has signage on site, just not installed.
  1. Machine room door has a Construction key core in the lock handle. Target will probably put their own core in the lock. The Inspector’s normally want a machine room key in the pit, recommend Brookfield Security have access to the key core to meet Inspector’s wishes. Target should make the key a submaster in that case.
  1. Terminate the camera coax on the car top, (opposite corner of the car operating panel) and in the machine room, needs to be in a separate electrical 4 x 4 box mounted on the duct, mark the cover as Camera so the Asset Protection does not have to go into the controller.
  1. Put the scanner in “fail safe” mode. Remove nudging on automatic operation so as long as there is an obstruction in the scanner, the gate will not close on automatic operation.
  1. Provide the MSDS sheets of the hydraulic oil in a plastic sleeve or folder mounted on at least one tank unit in the machine room.
  1. Provide a file holder per elevator in the machine room. Insert a complete construction manual of shop drawings, wiring schematics, test forms including the acceptance test, fire service test form, call back log, repair log and tasking log per specs

  1. Braille tactile is not required on a freight elevator. Currently is on entrance frame and Brookfield consultant wanted it replaced, am ok with it as is.
