In the event your sheep’s ID tag that is listed on the registration papers is lost, when you replace the tag you can note new tag information on this form. You may also use the form to add a Tattoo number or Microchip information or even to add newly placed Tag information to the registration certificate when there were no previous tags. Having matching identification information on sheep and on official Registry paperwork is important.

Return this form to the UHHSA Registrar when transferring or doing other registration work on your sheep and the new tag ID number can be corrected on your registration papers to match the new tag on your sheep. There is no charge when tag number correction is done at the same time as other work, such as transferring. If you want the new tag listed on your papers without other work being done the charge is $6.50 to reprint a new certificate. Remember to include the original certificate displaying the previous tag number.
