Taken from responses made by Community Southwark, Southwark Clinical Commissioning Group and Southwark Council to the Lambeth & Southwark Early Action Commission report ‘Local Early Action: How to Make it Happen’; as well as themes fromthe Council Plan

Creating resourceful communities

  • Community Southwark developing Community Action Network.
  • CCG programme structures for priority populations: children, complex adults & serious mental illness
  • CCG & council to support the VCS to ensure local communities aware of services available, and the most vulnerable residents are linked with mainstream provision.
  • SAIL (Safe and Independent Living) project delivered by COPSIN collaboration of VCS groups.
  • Council / CCG / VCS scoping social investment models.
  • CCG piloting empowerment models.
  • Improve outcomes for people affected by complex social issues eg domestic abuse, mental health issues & long term health conditions – and develop work with young people and older people.
  • Public Health working with partners on a council housing health needs assessment.

Responding to wider determinants of health

  • Council’s Fairer Future promises: free swimming and gyms; quality affordable homes; investment in parks & green spaces; more & better schools; nurseries & childcare; education, employment & training; Age-friendly borough and Southwark Ethical Care Charter.
  • Council , NHS and VCS joint working:
  • Supporting financial health eg financial health advice in health centres.
  • Promoting walking & cycling & use of green spaces; lobbying TFL to extend Ultra-Low Emission Zone to Southwark.
  • Stop Smoking / tobacco control initiatives.
  • Commitment to London Living Wage.
  • Promoting healthy relationships & awareness of warning signs of domestic abuse.
  • Community Southwark proposal to consider engagement with those who have relationships with the most vulnerable: landlords, gambling shop staff, GP surgery receptionists.

Stronger role for VCS in developing policy & strategy

  • Greater recognition at strategic level of the VCS’s contribution to meeting residents’ needs.

Services embedded in local communities

  • CCG Population Health Management Approach to ensure respond to needs on a highly localised basis.

Delivering system wide value – behavioural, cultural and systemic change

  • CCG mapping assets across boroughs.
  • CCG research challenge fund: to learn about VCS work with specific populations.
  • Consider Change Fund to kick-start systems change.
  • Pooling of resources between CCG and council.
  • Joint voluntary sector strategy.
  • Healthy High Streets initiative.

Shift investment into early action

  • Need for clear oversight arrangements to give strategic leadership for early action.
  • CCG Five Year Forward View to be published June 2016.
  • CCG & council to classify preventative spend, monitor and report on it.
  • Council working in partnership with Southwark Funders to ensure funding priorities reflect greatest need.
  • Southwark Giving Scheme being developed by council / VCS.

Commissioning at a time of diminishing resources

  • CCG & Council Commissioning Partnership Team to transform commissioning process to support early action & develop shared evaluation frameworks, improve efficiency, reduce duplication, reduce demand for more intensive interventions , and respond to changing needs rather than historic service models / silos.
  • Proposed creation of single commissioning unit – council / CCG.

Better partnership working, co-ordination & knowledge sharing and integration of VCS with statutory services.

  • CCG Local Care Networks – bringing together health & social care providers, statutory & VCS.
  • Consider Multi-Agency Working and Alternative Delivery Models – preventing individuals (council tenants) ending up in high cost social care.
  • Community Southwark establishing Provider Led Groups for VCS orgs to look at policy areas, independent of council & CCG.
  • Community Southwark: practical ideas such as ‘living noticeboards’.
