1. Trust’s priorities for 2013/14: We would like you, as one of our members, to help us plan our priorities for the coming year. Please rank the following 1 – 12 in order of importance, with 1 being the most important to you:

• / Further reduce avoidable harm involving things like blood clots, patient falls,
pressureulcers and surgical site and line infections / [ ]
• / Further reduce infections like MRSA and Clostridium difficile / [ ]
• / Continue to improve our clinical outcomes as reported on indicators that
measure the risk of dying when under the care of UCLH / [ ]
• / Improve how we communicate with GPs about the care we provide to their patients / [ ]
• / Improve our administrative processes so that patients can book appointments
more easily / [ ]
• / Improve patients’ experience of how they are looked after and treated when in contact with UCLH staff / [ ]
• / Make it easier for patients, GPs and others to get in contact with our staff / [ ]
• / Make UCLH a better place to work for its staff / [ ]
• / Reduce the time that patients spend waiting for decisions and treatment in A&E / [ ]
• / Reduce the time that patients wait for appointments and for treatment / [ ]
• / Continue to meet waiting times targets for cancer patients, including time to see a consultant for the first time and the time to get treated / [ ]
• / Wherever possible move the care currently provided in hospital closer to where patients live, for example to GP surgeries or other community health facilities / [ ]

4. The “Making a Difference Together” campaign is an initiative to positively transform patient and staff experience; attempting to make UCLH as well known for its patient and staff experience as for its excellent clinical outcomes, research and education. We would like members to be involved with the campaign so that the voices of our patients and the public we serve are represented at all stages of the campaign. If you're interested in taking part, please indicate as many, or as few, of the areas below that you would be willing to be contacted about getting involved with. Recording an interest in no way commits you to taking part in any events.

Surveys - We may from time to time send you short surveys to complete asking

for your views on how we should run certain services and projects.

Focus Groups - We may send you invitations to be involved with one off meetings

to get detailed patient / public feedback on specific subjects of interest to you.

Patient Panels - We may invite you to be involved throughout a project providing

feedback across the whole course of a project.

Reading Groups - We may invite you to provide feedback on new written material

produced by the Trust to make sure it was clear, understandable and has all the

content patients need.
