Brief Overview of the
Position: / To support the Co-ordinator in providing person centred support to older people with long term health conditions and in particular those with memory problems, that enables older people, their families and carers to maintain their independence and wellbeing; promoting choice, control and dignity, both in the community and in groups.
Main Duties and
Responsibilities: /
  • To undertake initial assessments and reviews to support the referral process and development of personal plans. Ensuring that outcomes are achieved both for individuals and the project and people are able to live well with their health condition.
  • Using person centred approaches ensure that all support and activities offered, including personal assistance and care, promote dignity, choice and control and are delivered in a sensitive, non-judgemental, responsive manner, tailored to individual needs.
  • Assisting the co-ordinator in developing and implementing a programme of activities and support options to meet goals identified in individual support plans; identifying opportunities to develop new and innovative support options.
  • To be aware of the needs of carers and develop a co-operative and supportive relationship with them.
  • Effectively communicating with our customers, their carers and families and maintaining positive relationships with other VCFS groups and agencies and liaising with other professionals involved in the care of older people.
  • Carrying out routine administrative tasks, accessing and inputting data, maintaining records and retrieving information - using the Age UK Lancashire CRM system as appropriate.
  • Supporting volunteers’ and enabling them to use their skills and abilities to their best effect to support our customers.

Age Concern Lancashire has been in existence since 1941, formally adopting the name Age Concern in the 1970s. In 2010 Age Concern Lancashire made a positive choice to become Age UK Lancashire, enabling the charity to be part of the wider network of Age UKs across England, Scotland, Wales and Northern Ireland.

We are a dynamic and influential Age UK organisation that provides services across much of the county. In addition we ensure older people receive information and support to make informed choices and are empowered to live their lives as independently as possible. We employ over 300 staff supported by 1,000 volunteers.

We offer a range of insurance and other products that are tailored to the needs of older people and from transactions, receive commission that enables us to have an office in the districts in which we work that provides a base for staff and a place for telephone and personal callers.

We have eight charity shops which sell a range of mainly donated goods, raising independent income for service provision.

Some services are paid for by our customers, in particular our Home Help service which we run as a social enterprise.

We are probably best known for our service provision and our focus is on working with our customers and providing a high level of person centred support in all that we do. Within our services teams, our main outcomes are Health & Wellbeing(H&WB), Social Inclusion (SI) and Promoting Independence(PI). All the services we currently deliver can be clustered into these three outcome groups.Health and Wellbeing is all about keeping people active, involved and enjoying life; Social Inclusion is about ensuring people can stay involved and are not marginalised in any way and Promoting Independence is concerned with preventing unnecessary hospital admissions, avoiding premature residential care and finding alternatives to dependence upon statutory social care services.

The project aims to coordinate and make more accessible a full range of services to meet the needs of all members of the community. Daybreak – Support worker (North)

To provide high quality person centred support to older people with long term health conditions and in particular those with memory problems that enables older people, their families and carers to maintain their independence and well being.

Work will include initial assessments and reviews, ensuring outcomes are achieved for individuals and the service and that people are able to live well. This will include delivering a range of activities and support in community settings and at a 1-1 level tailored to individual needs as well as administrative duties including use of the Age UK Lancashire CRM data base system.

Terms and Conditions

Post Title:Support worker- Daybreak

Location:Based in Lancaster, Fleetwood or St Annes

Grade/Salary:Scale 2pt 11 to 2 pt 13 £14,881 to £15,599 p.a.

Hours of Work:28 hours per week over 4 Days

Pension: As from 5th February 2014 the organisation’s Group Pension Plan is closed to new employees and will be replaced by auto-enrolment from 1st July 2014 for all eligible employees.

Holidays:24 days annual leave per annum plus 8 days Bank and Public Holidays, rising to 29 days plus Bank and Public holidays at the beginning of the holiday year after attaining 5 complete years’ service. Part time workers will be entitled to 24 days annual leave pro rata plus Bank and Public Holidays which fall on their normal working day

Travel Expenses:Travel expenses will be paid for all journeys required for work, excluding travel from home to base.The current rates are 45p per mile for the first 10,000 miles per year and 25p per mile thereafter.

The nature of the post will require travel across the county. A full driving licence, appropriate insurance cover and use of a car is essential.
