Qualification packets should include a brief description of your company, and why it is the most qualified for the design of two k-6 elementary schools and the complete modernization of the Centralia High School. You must show previous success utilizing the traditionaldesign, bid, build method of multiple projects simultaneously. Include demonstrated experience in design of new construction projects, and modernization projects. Qualifications packets should clearly show your firm’s ability to achieve LEED silver, or Washington States High Performance Building Standards. Additionally, the packet will include a portfolio showing examples of similar projects completed by your firm. Qualifications will also include references for us to contact for further information regarding your work. Submit four complete hard copy packets. Firms may also include pdf copies on a disc or USB drive.

•Cover Letter (onepage)
•Section 1: Identify Firm(s) including name, address, phone number and contact person regarding this RFQ (onepage)
•Section 2: Provide an organizational chart identifying disciplines, specific personnel, and role of those who will be assigned to theprojects
•Section 3: Describe your project approach for the projects (twopages)
•Section 4: Firm’s capacity (e.g. number of full-time licensed architects). Provide resumes and workload for people assigned to this project-refer to staff listed in Section 2. (Maximum one page perperson)
•Section 5: Detail firm’s quality control process with regard to design, document control, and construction administration (onepage)
•Section 6: Firm’s litigation history and ask, “Do you have any pending litigation or binding arbitration with a client?” (onepage)
•Section 7: Copy of your firm’s current professional liability or errors and omissions insurance (onepage)
•Section 8: List firms last five (5) similar projects including size, location, and contact person (maximum 5pages)
•Section9:Describewhatmakesyourfirmuniquelyqualifiedforthisproject(onepage)
NOTE:
The desire is for two almost identically designed k-6 facilities. The Districts desire is to save time and cost by matching the design. We understand that there will be differences due to site conditions, but desire to keep the overall building design as similar as possible.
