Submission form F – RATED CRITERIA (revised)

Proponents shall provide information required in this submission form as part of Stage II Evaluation of Rated Criteria. Stage II will consist of an evaluation of the rated criteria to determine the high score based on the following criteria. Below is an overview of the categories and weighting for the rated criteria of the RFP. Proponents shall refer to Section 3.3 - Rated Criteria for more information. Proponents who do not meet a minimum threshold of 70 points will not proceed to Stage III of the evaluation process.

It is required that proponents use Submission Form F – Rated Criteria, and no other format when completing the Rated Criteria submissions. Please complete Submission Form F – Rated Criteria as per the instructions contained therein. TCHC reserves the right to decline to evaluate any information for Stage II Evaluation of Rated Criteria which is not contained within this form.

1.Company Information and Experience(25 Points)

Submissions will be evaluated based on details of key personnel identified in the project specific organization chart: number of years in this position; experience; education; licenses along with Sub trades experience, education, training, licenses.

Proponents will explain the following evaluation criteria in your submittal of your CCDC 11.

1.1Proponents must provide the brief description of the proponent and description of goods and services the proponent has previously and/or is currently delivering, with an emphasis on experience relevant to the Deliverables. (4 points)

1.2Supplementary to CCDC 11 - Section 7: Proponents must provide resumes for key personnel (identified in Section 7) including principal in charge, project manager and foreman. (4 points)

Resume needs to include the following information for each person:

-Education

-Project experience

-Number of years of experience in Construction Restoration work

-Number of years with Proponent Company

-Type of training

1.3Project specific Org chart - a project specific chart showing the entire proposed team by position titles/roles for each proposed team member, number of employees for each position including any support personnel if applicable. (2 points)

1.4Supplementary to CCDC 11 - Appendix B - Proponent must select 3principle projects of similar construction work from Appendix B that were completed in the last 5 yearsand provide additional information below. Construction budget must have minimum of $750,000 per project. Please provide the details below maximum 2 pages for each project. (5 points for each project with total 15 points)

Submissions will be evaluated based on performance history of completed work with good workmanship and expertise in delivering similar size structural projects (windows/envelope projects)

2.Communication(10 Points)

Submissions will be evaluated based on the communications procedures and responsiveness to correct problems once identified:

Proponent must identify the communication protocol with owner and consultants regarding the proposed construction schedule and work details, including timelines for notices, etc, the communication steps during the construction, and responsiveness to clients.

3. Scheduling(25 Points)

Submissions will be evaluated based on the contractor’s understanding of the project, details, and approaches of scheduling, resource allocation, methods and procedures to complete the job on schedule.

3.1Proposed detailed construction schedule in Gantt Chart (20 points).

Gantt chart must include:

  • Commence date and commission date (5 points)
  • critical path, milestones, key activities, operation phases, project phases, substantial completion, final close out meetings, commissioning and any specific requirements within the RFP. (10 points)
  • Number of foremen and workers at site and how they are related to the activities (5 points)

3.2Provide a description of proponents’ methods and procedures as it relates to:

  • Ability to complete jobs on schedule and on budget (3 points)
  • Innovations on industry best practices (2 points)

4.Change Order Process(10 Points)

Submissions will be evaluated based on the approaches and strategies on how contractors manage change orders effectively with minimum or no interruption to the project.

4.1Describe the Proponent’s approach to the management of changes in the Work. (5 point)

4.2 Please submit an actual example from a previous project listed on Appendix B of CCDC11 for the following:

  • owner's changes (1 point)
  • unforeseen site condition change (2 points)
  • scope changes/engineering (2 points)

5. Quality Control(10 Points)

Submissions will be evaluated based on the quality control processes, supervision of the staff and sub trades and identification of deficiencies as following.

Provide a detailed description of the processes and procedures you would use to consistently comply with TCHC’s specifications. Your answer must include, but not be limited to, consideration of the following:

  • following the third party engineering instruction (3 points)
  • compliance to specifications, drawing and codes (3 points)
  • review of work and inspection (2 points)
  • supervision and monitoring work of in-house staff and sub trades (2 points)

6.Deficiency Control(10 Points)

Submissions will be evaluated based the approaches used by the Proponents to manage and rectify the deficiencies.

6.1 Describe the Proponent’s process for managing the correction of deficiencies. (4 points)

6.2 Provide examples of the tools that the proponent would use in the execution of thecurrent project to control deficiencies. (3 points)

6.3 Describe the tracking mechanisms used to close out deficiency list items. (3 points)

7.Health and Safety(10 Points)

Submissions will be evaluated based on whether the proponents have appropriate site specific health and safety plan and precaution measures in monitoring sites and preventing accidents

7.1 Submit Health and Safety Manual (as per specifications of Ministry of Labour). Bidders must submit the 1 copy of the company’s Health and Safety Manual with this submission otherwise the bidders will be disqualified. (1 point)

7.2 Describe the approaches to health and safety for the project. Make specific reference to the following items: (5 points)

  • Work in a building where multiple contractors will be working on separate projects
  • The building tenants and site specific measures that will be implemented to ensure that safety of the construction team, and building tenants during the construction period

7.3Describe the Proponent’s approach to health and safety for the Project. Make specific reference to the following items: Health and Safety Training, and tool box meetings (4 points)
