6.BRIEF RESUME OF INTENDED WORK:-

6.1. NEED FOR THE STUDY:

In Ayurveda, treatment comprises of quadrants namely, the physician, the medicine, the attendant and the patient.1,2Among these, medicine has the second importance next to physician because medicine makes the treatment possible.1Medicine has four distinct qualities. A substance is eligible to name as a medicine only if, it has the capacity to formulate into different forms, it has more than one attribute or, it can be administered to different diseased conditions, it is endowed with the optimum level of potency or, it is available in large quantity to meet the need of treatment and it should be in a useful state, i.e., free from any sort of infestation or, it should have the ability to cure diseases.3

A medicine can be made use in different diseased conditions.2There are more than one indication for almost all formulations explained in Ayurvedic treatises. Even though the formulations are explained in a specific disease-treatment context (roga-cikitsa-adhikara), the indications for one formulation may include more than one disease (roga). For example, Abhaya-arishta mentioned in Arsha-cikitsa has also indicated for the treatment of diseases like grahani, hrdroga, pandu, gulma, udara etc.4It is very common in practice that to prescribe the same medicine for different pathologies and it is seen to be useful also. But no valid documents are available till date for standardizing the prescriptions based on multi-indication effects of such medicines.

To effectively practice medicine a physician has to master all the relevant classical texts, mere understanding of one text is not sufficient.5The tool for decoding Indian scientific texts is known as tantra-yukti, atidesha is one among them. It explains the way of understanding similar but unexplained matter from other relevant contexts.6Detailed literary research shows that till date there is no direct reference with regards to the bahuguna of dravya-pada by applying atideshatantra-yukti in the field of researches in Ayurveda.7

Hence this study is planned to do a survey of prescriptions of practitioners representing all level of experience and practicing regularly on vata-vyadhiespeciallygrdhrasi. In this study multi drug prescription on the first visit with diagnosis is taken into consideration. This study intended to reveals that the pathogenesis (samprapti) is given importance in treatment so that the principles of intervention are based on dosha-dooshya-sammoorchana (sampraapti-vighatana). This will also validate the multiple indications of classical preparations. It can be a base for finding out the most common combinations in use.This study also planned to make use of the idea of atideshatantra-yuktifor literary application of bahu-gunaquality explained for medicines.

6.2 REVIEW OF LITERATURE:

Among cikitsa-catushpadadravya is the second significant factor because of its utility in treatment.1There are four characters for an ideal drug.Bahu-guna is one among them. It is explained in two aspects, as the fractional attributes of a drug like heaviness, lightness etc or the capacity of a medicine to cure more than one pathological condition.2There are a number of synonyms explained for dravya or aushadha. Medicine is broadly divided into preventive and curative (oorjaskaram, roganut).8The drug should be freely available because even if it is effective but not available it is of no use. The medicine should be useful for many disorders. It should also be suitable for the concerned disorder in terms of effectiveness. It should be capable of being subjected to various forms of pharmaceutical processing because one pharmaceutical form is not useful in all disorders nor is suitable for every patient because some are averse to juice and others to paste. Excellence(sampat) means that the drug should not be injured by external agents such as insects, humidity etc. and should be endowed with normal rasas. Gangaadhara clarifies it further that the drug should have grown (and collected) in proper season and thus should have full potency. Yogendranatha mentions the importance of suitable land.9

An ideal drug should be collected from a fertile and auspicious land on an auspicious day. It should be in optimum quantity, pleasing to mind and endowed with optimum attributes like smell, colour etc. Such a drug should be capable of correcting the vitiation of humors and should not produce any negative effects to mind or body when applied in judicious dose and time.10 Medicine is the instrument for treatment. There are various classifications for medicine/treatment on different criteria; for example providential therapy (daiva-vyapaashraya), logical treatment (yukti-vyapaashraya) and self restrained therapy (satva-avacaya) or treatment with substances (dravya-bhootacikitsa) and treatment without substances (adravya-bhootacikitsa).11

Grdhrasi is a vata-vyadhi with symptoms of numbness along with different types of pain like penetrating pain, cramping pain and pulsating pain. The specialty of grdhrasi is that these symptoms radiates from buttock region to pelvic girdle, low back region, thigh, knee joint, calf region and finally the foot.12It is of two types based on the predominance of involved doshas as vataja and vata-kaphaja.13

Medicines like Maha-rasnadikashayam, Mahanarayanatailam and Prasaranetailam in Sarngadharasamhita have direct indication of grdhrasi14; on the other hand medicines like Rasnasaptakam, Yogarajaguggulu are prescribed for grdhrasi even if there is no direct mentioning of the disease among the indications of these medicines.15

Tantra-yukti is the technical tool help to decode the Indian scientific texts which are mainly in the form of verses. Atideshatantra-yukti is one among them. It provides the idea of referring and understanding the similar but untold concepts in other treatises.6

6.4 OBJECTIVES OF THE STUDY:

  1. A critical analysis of formulations of vaata-vyaadhicikitsa from various classical texts of Ayurveda using atidesatantra-yukti.
  2. A cross sectional survey study on medical prescriptions of vaata-vyaadhiespecially grdhrasi for validating the multiple indications of classical medicines.

7. MATERIALS AND METHODS:

7.1 SOURCE OF DATA:

Current study is carried out in two steps namely

1. Literary study

2. Survey study

Literary study: The relevant references of formulations of vata-vyadhicikitsa from various classical texts of Ayurveda. The compiled materials will be reorganized and critically analyzed.

Survey study: Collecting the digital photography of prescriptions from selected OPDs and practitioners with the diagnosis and medicines. Distributing a prospective questionnaire with specific format among the practitioners to know the intention of those prescriptions.

Methodology of Survey study:

Sampling frame: The sampling frame consisting of a convenient sample of Ayurvedic practitioners of a minimum sample size of 250 representing all level of experience and practicing regularly on vata-vyadhi cases especially grdhrasifrom Karnataka and Kerala.

7.2METHOD OF COLLECTION OF DATA:

a)PLAN OF THE STUDY:

  1. The prescriptions of doctors will be collected from the selected OPDs by convenient sampling.
  2. The digital photography of prescription will be taken.
  3. The prescriptions will be kept anonymous, i.e., without revealing the identity of patients as well as practitioners.
  4. The prescriptions of vata-vyadhiespecially grdhrasi in which the diagnosis have been clearly mentioned will be taken.
  5. It should be a multi-drug prescription given on the first visit.
  6. The frequency and indication of these medicines will be analyzed.
  7. A prospective questionnaire will be distributed among these doctors and their explanation about the prescription is collected.
  8. Whether these practical indications are in tune with the classical indications or solely based on the experience of the practitioner will be analyzed.

  1. A convenient sample of physicians representing all levels of experience and practicing regularly on vata-vyadhiespecially grdhrasi.
  2. A prescription on the first visit of the patient.

  1. A mono-drug prescription
  2. A prescription containing repeated change of combination
  3. A prescription without the demographical data of the patient
  4. A prescription with more than 5 formulations
  5. A prescription without diagnosis
  6. A prescription in which the diagnosis is anything else than grdhrasi
