I. POLICY

The City grants employees a Personal (unpaid) Leave of Absence.

II. SCOPE

This policy applies to all employees.

III. GENERAL PROVISIONS

A. An employee requesting a Personal Leave of Absence must make a written request to their supervisor for consideration. The supervisor should forward the request to the city administrator for approval. A Personal Leave of Absence must be approved prior to the start of the leave, whenever practicable.

B. An employee on a Personal Leave of Absence has no guarantee of a position with The City.

C. A Personal Leave of Absence may be approved for a maximum period of six (6) months.

D. The City reserves the right to deny a request for a Personal Leave of Absence.

E. Personal leaves are normally granted for the following reasons: family responsibilities, personal illness/injury, personal emergency, education or extended travel.

F. Personal leaves are not granted for purposes of obtaining other employment, relocating, or extending the date of termination. Employees who accept employment while onpersonal leave will be considered to have voluntarily terminated employment.

G. An employee may be placed on a personal leave of absence by the employees’ supervisor with approval of the city administrator if an employee's approved paid

leave is exhausted and the employee is unable to return to work or if the employee's situation warrants such action.

  1. Insurance benefits are not paid during a Personal Leave of Absence. If an

Employee wants to continue those benefits eligible for continuation under COBRA, the city administrator should be notified. The employee is responsible for paying the full monthly COBRA premium for each benefit continued.

  1. Paid leave benefits are not accrued while on a personal leave of absence.

Employees have the option of having their full PTO balance paid out at the commencement of their personal leave, or upon the completion of the leave if their employment is terminated at that time. Employees may not use PTO on a regular or periodic basis while on a personal leave.

  1. It is the employee's responsibility to notify the city administrator of their

desire to be considered for available positions at least three weeks prior to their return date. The employee must apply for positions based on qualifications and work record. If a position cannot be found before the leave expires, the employee will be considered to have voluntarily terminated employment.
