Study Guide for Alteration in Hematologic Function

STUDY GUIDE FOR ALTERATION IN HEMATOLOGIC FUNCTION

As you study for this chapter, you will recognize many of the disorders as occurring both in adults and children. Some of this content you will have already studied in previous courses. Please take time to review your notes from medical-surgical nursing and pharmacology, as well as previous anatomy and physiology courses. You are responsible for the pathophysiology of the diseases discussed in this chapter, although time does not permit me to review it in detail during class. If you have questions about the pathophysiology, please see me before class, so that you will be better prepared to understand the discussion in class.

Students are responsible for the following medications:

Ferrous Sulfate

Hydroxyurea

DDAVP

Amicar

Deferoxamine

Additional recommended activities to assist students with the content:

  1. Go to www.cdc.gov/ncbddd/sicklecell/quiz and take the quiz about sickle cell disease. There is a link back to an article with additional information if you were not able to answer the questions correctly.
  2. Go to www.yourgenesyourhealth.org/sickle/have.htm
  3. On the left hand side of the page is a menu. Scroll down to “What is it like to have it?”
  4. Roll over the titles to the right to view video clips of a patient as she describes situations related to dealing with the pain, school, having fun etc. Think about how the nurse can assist this patient deal with sickle cell disease as a chronic disease
  5. Define the following terms:
  6. Hemarthrosis
  7. Hemochromatosis
  8. Hemosiderosis
  9. Pancytopenia
  10. Physiologic anemia of infancy
  11. Pupura
  12. Reticulocytes
  1. Review the chart for normal blood values for children. Your 2 year old patient has the following values. What would be your diagnosis based on these values?

TABLE 1 BLOOD SAMPLE RESULTS
Red Blood Cells Count / 3.5 million/mm3
Hemoglobin / 7 g/dL
Hematocrit / 30%
Serum Iron / low
Mean Corpuscular Volume / low
MCHC / low
Total Iron Binding Capacity (TIBC) / high

What signs and symptoms would you expect this patient to display?

What is your treatment plan?
